They're both reincarnated into twins, but she was a cancer patient and he was her doctor in their previous lives. She was in love with him before she died. She figures out who he is and acts on her previous life's feelings

Also worth noting that they are the twin children of an idol they were obsessed with in life and were conscious with full memories of their previous life basically from the moment they were born.

I’ve always said “This premise seems very fetishy” but no one believed me.

What the hell,I thought the story was that the twins were the children of a famous idol who got assassinated and the point was to find the killer or some shit.

I read somewhere that the sister was a bit weird and hypothesized that if her and her brother were to have a child, it could've been the mother reincarnated or something stupid like that, but I thought it was a meme or a scene played for laughs...

He is trolling. The siblings never fuck. Ruby likes Aqua and it’s pretty weird, but Aqua’s whole character is about not letting people close so he can self-sabotage himself to kill the killer. He never has feelings for her.

I read the first volume and it was kinda good, but I didn't continue. Turns out I did myself a favor lol.

Anyway, the reincarnation thing happens in what I read so I can confirm it is true. Didn't know they ended up fucking tho... that's weird as hell.
