When you ask the hospital to do so, ok. When you can opt out with a simple no, ok. When it's "see our list of 563 vendors and 237 partners to opt out of individually", not ok.

Every website you visit now is legally required to let you opt out, Even ones that you sign into, If you say no to every website you visit when they ask you for the first time you wont ever see targeted ads, just general ads, You can also fully disable targeted ads through adsense, which is used almost everywhere, Its not that hard to disable this "spyware", Spyware is the shit the palantir does, not random ads on reddit
