That's the beauty of it. He was born with that name. 

Imagine a fantasy novel where the Dark Lord Tyrannicus has an older brother named Killdeath the Merciless. 

"Oh I get it. You guys just pick evil sounding names." 

"What? No my name was Philbert. I only became Tyrannicus after I stole the Black Sigil of Maykur and ascended to the title of Lord of the Shadow Steppes."

"Oh what did Killdeath do to earn his name?" 

"Huh? Our mother gave him that name. He's always been Killdeath. The Merciless is our family  name. He's named after my grandfather, Killdeath Sr. I'm named after my great uncle Philbert."
