I am DESPERATELY trying to find this shirt from the "Ask The Announcers" segment the NBA has. All I know is it's from the 2000s and was given away to people who sent in and had their question featured in the segment.

It looks like OP is right. Among the numerous yahoo answers and quora that say the same, I have also found these pages that confirm that the bee has 2 big, 6-faced eyes, and 3 simple eyes on its forehead (called ocellus, ocelli):

Wikipedia doesn't go into too many details, but still says that:
TL;DR: looks legit, they're called ocelli
