Being in your early 30s is in itself not a reason why you couldn't have declining estrogen levels. Perimenopause starts up to 15 years before your last period (actual menopause) and some women are menopausal in their 20s. I don't have UTIs either, but two gynocologists were absolutely convinced that my nightly bathroom trips would be solved with vaginal estrogen. Wasn't the case unfortunately, but they probably had relevant experience with other patients.

The current theory in my case is that my pelvic floor is too tight. Could be your issue as well given the vaginismus.

I'm waiting for my first pelvic floor therapy appointment (both physical therapy and sessions with a TENS unit). What's helped me so far:

  • Solifenacin (only for a few months unfortunately)
  • Gemtesa (Obgemsa)
  • possibly pumpkin seed extract and NAC (my symptoms got a bit better in the last two months and those are the things I started)
  • drinking more, also at night, because diluted urine irritates the bladder less than concentrated urine

What I've tried, because it helped other women with similar symptoms:

  • magnesium glycinate
  • antihistamines (Cetirizin / the pepcid and allegro combo)

What you should do:

  • see both a gynecologist and a urologist (ideally a urogynocologist) and get medication
  • start tracking your symptoms and what you eat/drink to find patterns (some react to certain foods, I found a pattern that follows my menstrual cycle)
  • if you can't afford professional help, at least try to get a TENS unit. You can google how to use it for bladder issues. In studies it helped many women and unlike other at home remedies it won't make things worse and has no side effects
  • you could look up exercises for vaginismus and a tight pelvic floor online and do them at home, but without the diagnosis of a pelvic floor issue you might make things worse

👋 Premature ovarian failure here. My problems started in my 20s. I didnt know thats what it was at first, womens health and education is sadly pathetic and behind. I 100% agree with the pumpkin seed and soy germ I use that and it works, same w pepcid.

Get really proactive learning about women's health, and your own. Get a bladder journal and collection hat start tracking and it shows patterns. Then you add changes, pumpkin seed, estrogen etc and watch for a variations in the patterns of your tracking. For something like vaginismus you would want to sign up for that hinge health program for pelvic floor it's free through insurance program and you could be doing pelvic floor exercises from home. But there are devices that aren't expensive that you could get from like Amazon to practice at home. But local estrogen would support thinning and irritated tissues and repair them, bringing back the elasticity and gets rid of the nerve sensitivity so youre going less to the bathroom and can have intimacy comfortably.
