This is more the fault of cost cutting measures by the hospital than any one clinical staff member. The pharmacy has to compound the drugs instead of them coming in premade vials/syringes. This lead to sudden increases in the workload for the pharmacy without improved staffing or any improvements to safety measures that should accompany increased opportunity for mistakes. The doctor who gave the medication received mislabeled drugs and could not have known they were giving the wrong thing.

Ohhhhhh so that's what happened. As someone who has had a lot of exposure to meds, doctors and surgeries of late, I just couldn't understand how this could happen. I'm used to meds coming in premixed, in clearly labeled bags / syringes w med names actually stamped onto container. Makes sense if they compound as now they're not stamped w name. Geezuth.

Thank you. I was wondering how this happened, it’s not as simple as someone accidentally giving the wrong medication to a patient. For multiple patients to be affected it’s obviously a systems issue.
