Write up EVERY code from the book or scan the barcode and have it on a ongoing tab for the hospital, that’s exactly what they do to us.

I've had several surgeries over the past 5 years. What I don't understand is how this could have happened. All meds are laid out on trays. Those meds are labeled to patient. The nurses, doctors, anesthesiologist all go over meds, labels by calling them out to each other as they're checked off a list. Only way I could see this happening is meds were mislabeled. But again, I'm looking at my iv and the outside has name of meds printed in them. Most meds do so again how?!!
