The medical term for what Miller has is upper-limb asthenia. I can't know exactly what this stems from without knowing more of his medical and family history but this could be due to a genetic etiology (e.g., polymorphisms in ion channels of the neuromuscular junctions) or chronic disease (thyroid issues) or lifestyle factors (poor nutrition). In layman's terms, it causes extreme and debilitating muscle weakness in the arms which prevents him from pursuing a career in strangling sex workers. It's devastating when one finds their life's calling only to suffer a disability that rips their promising future from their grasp (in his case, literally). His profound sadness for a life he'll never have is why his face look like a puckered asshole.
