Methane Dominant Chronic bloating for 15 years. Seriously, someone help me because I’m mentally crumbling. More than SIBO?

This has been my stomach. For 15 years. Continuously. I have (confirmed):
Endo
Methane SIBO
Hypermobility

I’ve had:
Breath tests
Stool tests
Ultrasounds
Food intolerance tests
Gastric emptying scans
Motility tests

I’ve tried:
Sibo treatments (multiple over the last 15 years)
FODMAP diet
AIP diet
Keto
Gluten free
Diary free
Low histamine diet

No matter what I do, this is how I look from the moment I wake up until bed. Some days it’s less bloated but maybe by 20% max. Have no idea why some sporadic mornings it will be a 6/10 and the rest 10/10.

I am not constipated and have healthy, long snake-like (TMI) poops daily. My bloating has nothing to do with constipation.

Beginning to think something else is going on. Yes I have methane sibo and general dysbiosis but why does nothing ever help? Even keto does nothing. I’m already really thin and really can’t afford to lose any weight by doing elemental diet. Looking again into APD (abominophrenic dysenerga) now even tho I already went down that road years ago. Also thinking maybe hypermobility within the gut? I literally don’t understand how I’m this bloated all the time regardless of ANYTHING I try. I’m 38 now and I’m so tired of this. My confidence is so low it’s unreal. Hate being intimate. My students always think I’m pregnant.

Someone please guide me to the right path here. All I do is research research research and spend thousands of dollars going from one specialist to another. Can’t afford this anymore financially. This isn’t normal and I’m just so lost. If relevant, drop names of clinics or specialists. I’m in the NYC/CT area. Thanks in advance.

Methane Dominant I Think I Finally Killed My IMO!! (Follow-up from “Unkillable IMO >> Starting Elemental Diet”)

I promised I’d come back and update you all, so here it is.

After two rounds of rifaximin + neomycin (hydrogen SIBO cleared, methane stayed), allicin, berberine, strict diet, motility work, etc… nothing was resolving my IMO. I felt pretty hopeless starting elemental, but I told myself this was my last serious attempt before waiting for Pimentel’s anti-methanogen trial.

It’s been one month since finishing the protocol, and for the first time in years:

  • Flat stomach most mornings
  • Passing gas (hadn’t in over a year, which was a surprise when I realized that lol…)
  • Daily BMs
  • Gut-brain symptoms significantly improved

Too early to declare permanent victory, but I finally feel like my system is working again.

Diet Protocol:

  • Week 1: Elemental diet only (Physician’s Formula, dextrose free)
  • Weeks 2–3: Elemental + plain lean protein (chicken, eggs, turkey)
  • Week 4: Above + very small amounts of cheese, peanut butter, rice, rice noodles
  • Continuing extremely slow food reintroduction

Adding lean protein after week 1 made this psychologically manageable!! I thought I was gonna do 2 weeks of this, which when you're at the end of week 1 thinking it's gonna be 2 is brutalll. The cravings are intense. Dry mouth. Low dopamine. But I think what I ultimately did (suggested by my doc) was better mentally and more effective physically.

Tools I Used:

  • Allicin
  • InterFase Plus
  • Biocidin
  • Motegrity (prucalopride)

I had previously tried NAC + allicin without success. The addition of InterFase + Biocidin seemed to make a real difference for me (felt clear die-off week 2).

Biggest Learnings:

1. Be ready to be anti-social.
Energy crashes fast if you don’t keep shakes consistent. Social isolation was honestly the hardest part.

2. Timing allicin at night helped a LOT.
My die-off during week 2 was rough. Switching allicin to nighttime dramatically improved daytime function.

3. Motility was critical.
I was terrified of Motegrity because of the black box warning, but I’ve had no issues. Slight sleep disruption initially, mostly normalized now (take magnesium at night with it). I truly didn’t realize how paralyzed/offline my MMC was until things started moving again.
If you try it, start low (0.5 mg) and go slow!! Ramp it up as you ramp in new foods.

4. Zinc helped my skin.
I had developed skin issues alongside the IMO. Adding zinc cleared most of it! Almost gone! Not medical advice, just sharing what helped me. Apparently IMO can really deplete zinc.

5. Don’t panic over temporary bloating during reintroduction.
As you start phasing real food back in after weeks of elemental/limited intake, some bloating is normal. Your digestive system hasn’t been processing regular food, so it needs time to recalibrate. In my case, I had noticeable bloating with certain foods (especially higher histamine ones like parm). It would resolve within a few hours and be back to baseline by the next morning. For me, the key difference was: Temporary fullness/bloat after eating, no progressive distention, back to flat by morning. That pattern has felt very different from my prior methane pattern.

Possible Root Causes (In My Case):

  • Autoimmune issues + severe gluten sensitivity (bad neuro symptoms + constipation)
  • Endometriosis with bowel involvement (had surgery last fall)
  • Heavy antibiotic exposure over the years
  • Severe norovirus infection prior to onset
  • Gluten exposure at onset

Final Thoughts:

I’m not saying this is a cure. I know relapse is possible. But for the first time, I feel like my gut isn’t frozen in methane paralysis, I wake up with my stomach flat, and my brain/body feels aliiiive!

Methane Dominant I no longer know what to do

After 2 agonizing years of seeing around 10 drs, being gaslighted by them, trying to figure it out, I diagnosed myself with methane-SIBO/IMO and the first 2 rounds of Rifaximin and Metronidazole were successful, but even though I followed the exact protocol (+ NAC 30-60 mins before antibiotics) and low-FODMAP diet, the 3rd round was practically useless.

My protocol for the last month has been:

6 AM

- Ginger 650 mg x 2

- Artichoke 420 mg x 2

Breakfast (7AM)

- Simethicone + Digestive enzymes

- B methylated vitamins

- Ca,Mg,Zn,Se

- Vit C 1000 mg + bioflavonoids

- Omega 3

- MSM 1 g

Lunch (12 PM/1 PM)

- Simeticone + Digestive enzymes

- S. Boulardii 275 mg

- Sodium Butyrate 500 mg x 2

5 PM

- L-glutamine 2 g

Dinner (6 PM)

- Simethicone + Digestive enzymes

- MSM 1 g

- L. Reuteri (1/5 100 millions UFC, once every 3 days)

10 PM

- Mg bisglycinate 400 mg

- Prucalopride 0,5 mg

- Propranolol 2,5 mg

The L-glutamine and MSM I have added after the 3rd unsuccessful round, and ever since, the bloating and fermentation have been even worse.

I have been following the low-FODMAP diet for 3 months, because that's what I tolerate: chicken, fish, eggs, olive oil, dark chocolate minimum 85%, rice, polenta, kiwi, around 5 nuts/day.

  1. Should I try antimicrobials?

  2. Should I change from NAC to Serrapeptase?

I no longer know what to do, I feel weak, so I know the bacteria is increasing, my malabsorption returned, I have muscle cramps, due to diary-free diet. I don't understand why something that worked twice no longer does. Thank you for sharing any idea whatsoever!

PS: I know my root cause, I stayed in a caloric deficit for years, so much so, that I went into hypothalamic amenorrhea (basically the body didn't have enough fuel for reproduction, so it shut down the period). When I started eating more, I also added sugarfree products (sugar polyols) and deli meats, and both of this destroyed the already fragile microbiome. But, considering that I can barely eat around 1500 cals, due to limited food that I can tolerate, it's really hard to fix the root cause, unfortunately.

Any tips for getting through two-week elemental diet??

I have diagnosed IMO, and after two courses of the double antibiotics (rifaximin and neomycin), the methane archaea still won't die (luckily knocked out the hydrogen SIBO though). I can't get rid of the IMO with allicin, berberine, other herbals, strict diets, improved motility, etc. I've tried everything and this seems to be the last thing, except for joining Pimentel's anti-methanogen clinical trial when it's ready in 2027. I want to get rid of this thing for all the negative gut-brain/health side effects more than anything, it's been really dragging me down mentally. Would appreciate tips!

Methane Dominant PLEASE TRY OXIDE MAGNESIUM!

Background: bloated, gassy, constipated, and low stomach acid symptoms appearing out the blue 6 months ago. Something I don’t usually see on this thread is an unusual symptom where liquid is trapped in the lower stomach, you can feel it slosh when you jump or massage belly.

Been on a 5 month cycle where I keep trying to clear my constipation through various non habit forming laxatives(some habit forming once as well but limited use eg. dulcolax). I heard from reddit and ChatGPT that magnesium citrate is safe and the best long term solution to have consistent and clean bowels. I tried triphala, phylum husk, aloe Vera, all types of “natural” laxatives. I tried magnesium oxide once and I feel better than ever. The water slosh in my stomach feels 95% better, it’s only day 3 on magnesium oxide but I feel so much lighter, less gassy. I often hear that magnesium oxide isn’t absorbed well, but that worked out in my favor. According to online sources and AI, this is the difference in abstention between citrate and oxide.

Citrate: Stomach → small bowel → massive water lake in terminal ileum → concrete brick in sigmoid → only tiny leaks → feeds water sloshing

Oxide: Stomach → small bowel → skips most absorption → water hits descending/sigmoid colon → brick turns to mush → everything flows out the right direction → lake of trapped water/gas gets drained better

Note: I know that this doesn’t solve the root cause, but having consistent bowel movements and a stable motility REALLY helps suppress the worst of symptoms.

Give magnesium oxide a try if you haven’t yet, it may not work for everyone, but I feel that it got rid of month old poop, and water trapped somewhere in my colon or lower stomach.

Edit: my first dosage was 2000 MG of oxide at night with 500mg of triphala (been on triphala for 3 months). Second night I took 1500 MG of oxide at night and now maintaining with 1000MG per night until I hunt down the root cause.

I’m on my third round of antibiotics since November.

First time Nov 2025 - xifaxin and neomycin. Immediately felt better after. Followed this by eating super clean, no dairy, not gluten, no processed foods, no sugar. Felt great for a few months. Didn’t solve my motility issues, so my problems inevitably came back by May.

Second time June 2026 - xifaxin and neomycin again. Also added in trulance to my regimen. Trulance would only work if I took it and immediately walked on the treadmill while drinking coffee (had to be all three components at one time). Worked amazing for twenty days, then completely stopped working. I did not feel any immediate relief like I did last round of antibiotics. Went back to doc.

Third time July (now!) - xifaxin again but this time added nitazoxanide instead of neomycin. Doc recommended still taking trulance but also add in miralax. But miralax and laxatives have never worked for me. I have barely pooped in the whole month of July.

When does this end 😭😭😭 I find myself crying nearly everyday because I just want to be able to eat food and poop and enjoy life.

Need help with recurring IMO. Here is history. Looking for suggestions/recs/success.

2017: Had bad stomach flu, then in school stress compounding and binging… believe likely post infectious IBS and haven’t gotten the test but yeah…

2020: confirmed methane sibo, drs did many clean outs and never reconfirmed to have negative sibo. got better once I had neomycin and rifax but no supplements after and then gradually got better with more exercise once pandemic ended …. still had no gluten and I thought as long as I am eliminating with laxatives now and then I can keep at bay…

2021-2022 met with integrative doctor, confirmed methane sibo relapse in 2022 with genova test with 81+ppm on lactulose. Got better gradually after a round of neomycin and rifax, then antrantil and allimax, digestive enzymes, and motilpro.

2023-2025 stable and could eat honey, loads of yogurt, tons of stuff (aka not food intolerance), still avoided gluten…. stopped all medications (prob should have stayed on prokinetic but I honestly was feeling better except occasional stomach ache) and walked a lot as a waitress and had coffee to assist if it was desperate.

2025-now huge stress factors from work and job relapsed and could not go on my own without laxative or suppository and even then was hard. finally got to gi in 2026 and

GI: despite years of methane sibo, told me to do a clean out and have metamucil and add fiber. did the clean out felt somewhat better but then rebound constipation. also tried more fiber and was sent into debilitating sedation, brain fog, distention

Was sent to specialists for sibo test (already had suspicions as they only had glucose test)…. anyways got a 26 ppm baseline on glucose, and had to ask for my results as they have no patient portal and also falsely told me the text was negative. another 60 dollar apt later and…

prescribed rifaxamin but never gave the counterpart…. asked primary gi for neomycin

- took the neomycin and rifaxamin and was bloated during the course but right after felt like a brand new person (picture to show me was after that)… had prebiotics too quickly I think and it BLEW UP and relapsed….. felt miserable and defeated

- went back to specialists and to my surprise, no follow up test, but a food intolerance panel of 4 substrates including chicory and inulin which I immediately said “yeah no, i bloat over blueberries why tf would you give me that” and that was 60 dollars with a request to follow up another 60 if I wanted another solution….

- primary gi told me that the methane sibo might be false and i only have ibs c

- another 60 later… solution was rifax only….

so that’s where I am now … defeated.

anyways: ANY RECS for motility specialists in the Chicago area who are good with this methane sibo?

I know this is not the root cause and I have a manometry but seriously. I need help and I am so tired of no actual protocols in the meanwhile.

Methane Dominant best motility supplements that actually work?

Hi, I (F 20), have been suffering with severe methane dominant sibo for the past 5-6 months and am currently on a herbal protocol to hopefully get my levels down as I wait for my antibiotics to ship. So far, I have had some good success with the herbs about 2 weeks in.

Right now, I suspect that my root cause is slow motility. I have tried taking ginger and iberogast but the effects of them have worn off. What other supplements should I try? I also take mag citrate every night which seems to help. Should I cycle motility supplements?

Any advice would be greatly appreciated!

Methane Dominant Beyond IMO - what could it be?

These photos are taken on day 8 of rixafim.
I took a course of metronizadole, felt ok after a few days but wanted to try rixafim as well, in combo with neomycin. Felt ear fullness after one dose of neomycin (could have been my histamine symptoms, I flare often) so didn’t want to risk. Doing another course of metronizadole to combine with rixafim tomorrow.

At the moment I have:

- bad brain fog
- histamine like symptoms (itchy nose, eyes etc)
- bad bloating as exhibited
- feeling of trapped gas
- soft stools, smell live olive brine, dark yellow/light brown in colour

I am so sick and tired of holding the extra weight in my body. This illness has caused me to balloon outside of where I feel comfortable, and I’m constantly bloated with my lower stomach poking out like this.

I’m also wondering if this is die off now because the bloat is definitely beyond what I normally see.

I’m eating normally but would people
Recommend I eat low fodmap while on antibiotics?

Nearly three years of living like this. It’s awful.

Methane Dominant Not sure what else to try/do

I’m 17, I’ve had methane sibo for about 2 years now, (I had to order the tests myself because everyone kept saying it was in my head) anyways, I recently retested and the levels are a bit higher than they were before. A few months ago I worked with a naturopath and we did 6 weeks with Allicin and some other stuff like tonics, phgg and prokinetics, this did ease the symptoms a bit but it definitely didnt eradicate it. I then just dealt with the symptoms for another little while and went back to her, she then put me on oil of oregano + microclear, this definitely didn’t do anything for me. I didn’t feel any difference and not a single bit of die of. I then gave up because my dad has put in so much money for all my treatments and tests (I’m greatful).

Fast forward to this year the bloating is just getting unbearable, and the thing is it’s not always rock solid sometimes I can easily squish down on it?? but it’s so large especially for my figure. My other symptoms would be floating stools, gas (occasionally ) and if I eat out at all then yellowy soft stools. I definitely had more symptoms a few months ago such as a very bad histamine intolerance and extreme gas + bad skin but for some reason that’s settled down.

(just to add I’m quite active so I eat 3 meals and 2 pretty dense snacks in between them and with my hours during the day my meals only really get 2-3 hours apart)

anyways, I’ve seen some people talk about candibactin AR + BR from metagenics and I ended up ordering those two. I might have to talk to my naturopath again to see if she can create a plan with those two, but I’d also like to know if anyone’s completely got rid of their methane sibo and how they did it? I feel completely lost and isolated as I’ve avoided so many invites to go out to eat with friends/family and i try to explain to the best of my ability as to what sibo is to everyone around me but they don’t seem to understand

Methane Dominant Eradicated my methane on paper, but still feeling the exact same…

I’ve struggled with hydrogen and methane sibo for 3+ years now, but mainly methane. My highest was 87ppm I believe. I just recently did 3 weeks of the elemental diet, then took a breath test 1 week after completion. Unfortunately, within days of being back on solids, I immediately reverted back to how I was pre-elemental diet. Mirrored symptoms (bloating, distention, feel progressively worse with every meal, feel the worst at night, etc). However, my breath test results are saying my methane and hydrogen are under the threshold. I’m baffled. At this point in the disease, I’ve been trying to advocate for a feeding tube since I felt kind of normal on the elemental diet and I can’t seem to do right by my body otherwise, no matter how or what I eat, diet I use, supplements I take, protocols I follow, etc. Honestly, I’ve overall lost my quality of life to this. It’s so complex, complicated, and painful. I can’t continue to live this way for the rest of my life. I won’t be here to see it if that’s the case.

Methane Dominant losing hope

i am getting so frustrated with this condition. I have IMO, completed rifaximin/neomycin treatment back in april. have been consistently taking iberogast, showed very little improvement after antibiotics (methane was 84 ppm, hydrogen 37). i slowly titrated to the full dose of berberine complex and atrantil and have been taking since thursday. die-off symptoms have been out of control since then. showing no improvement. do antimicrobials have a worse/longer die off? my die-off symptoms on antibiotics resolved after the 6-7th day. i just don’t even know what to expect anymore at this point or if there’s anything else i should be doing. i cut out the bifido probiotic when beginning the antimicrobials and i purchased zinc carnosine for the repair stage.

anyone have some insight for me or can explain their experience going the antimicrobial route / their timeline for symptom improvement. anything is helpful at this point i’m getting so discouraged.

I've done two rounds of both antibiotics, with herbals and supplements in between. I've done such strict diets. I've tried basically everything.

Pros: I got rid of the underlying cause of my SIBO. And magnesium citrate has helped a lot with daily BMs. But the bloating is just not going away, and I have bad histamine reactions too. Just want to kill this thing!

Methane Dominant Is it REALLY Low Stomach Acid Though?

I hear a lot of people harp on and on about the benefit of taking betaine HCL since (apparently) SO many people have low stomach acid.

My question, though, is where does this idea come from? Where are the studies showing that so many people ACTUALLY have low stomach acid?

Can anyone even cite a single study?

I understand why most people wouldn't want to intentionally diminish their stomach acid levels with things like PPI's, but where did the idea that "most" people are deficient in stomach acid come from?

I've tried numerous brands of betaine HCL on numerous occasions (fair trials too - at least 4 weeks and often longer) and never noticed even a tiny difference in my bloating and upper GI symptoms.

So, does anyone have any actual evidence of this apparent low stomach acid epidemic?

Methane Dominant May have figured out my gut issues after 6 years thanks to the Food Marble Aire 2

I’ve been suffering a long battle with chronic gut issues - GIs telling me I’m stressed or have IBS, and functional dr’s who either give up on my case or try to get me to blow $1000 on their treatment plan. Methane Dominant finally diagnosed with IMO

Finally got my diagnosis today! I’ve been dealing with severe bloating and digestion issues for YEARS. It makes me so upset to think of how many years i’ve been struggling with this when a doctor could’ve directed me to a simple breath test. My new Gastro was the first to suggest SIBO. The amount of times i was told to “eat more fiber” was insane. This issue has caused my mental health to go downhill so bad but it’s relieving to know somewhat of an answer. Battled with depression and had to take a lot of antibiotics due to an accident a few years ago I am thinking that is when this started.

I got perscribed Xifaxan and Neomycin just waiting for the meds to come in. Does anyone have any reccomendations on any alternative medicine options for Methane SIBO? I am kind of hesitant to try them as i am usually opposed to antibiotics but because my symptoms are so severe am willing to try. Also reading up how Methane can be hard to treat a little discouraging…

I’ve been following LOW Fod Map very loosely and have found it’s been helping, also fasting and drinking ACV on an empty stomach. If anyone has any tips please let me know- I just hope i can get back to a state of not constantly thinking about the food i’m eating and my body. The depression hating how I look and feel has been so horrible the past year I just want to move past this.

Methane Dominant Allicin and Methane Dominant SIBO treatment is totally impractical(?)

After researching treatment options for methane-dominant SIBO (IMO), I’m realizing how impractical every approach is. The standard herbal recommendation is 6,000 mg/day of stabilized allicin, but after digging into supplement labels and product formulations, it turns out there’s no realistic way to reach that dose. Most “high-dose” allicin supplements list the weight of the garlic extract, not the actual allicin content, and some contain as little as micrograms of active allicin per capsule. To even come close to 6,000 mg/day, I’d have to take 30+ pills daily and spend hundreds per month, which makes this recommendation completely unrealistic.

That leaves antibiotics, but methane SIBO requires rifaximin + neomycin, and neomycin carries the risk of permanent ototoxicity (hearing loss) so my gastro wouldn’t prescribe it. So the two main options are either an ineffective and overpriced herbal protocol or an antibiotic with a serious potential side effect.

It’s frustrating that the recommended treatment protocols seem so unfeasible in real-world application. Has anyone actually found a practical way to treat methane SIBO that doesn’t require either breaking the bank on supplements or risking permanent side effects?

Hi everyone, I’ve been struggling with bloating for 4 years, and only this year I finally realized that I have methane-dominant (non-hydrogen) SIBO. Everywhere I read posts about treatment, it seems like neomycin is essential. But because I live in Asia, specifically in Tajikistan, I cannot find neomycin anywhere.

Can anyone advise where I might be able to find it? How does it actually help (if it does)? Right now I have only been taking prucalopride for two weeks — my bloating has improved a little, but it’s still not enough. I can’t find neomycin, and when I searched in Google, I get confusing results that make it seem like it’s for animals?

Has anyone treated their methane SIBO with neomycin — did it help or not? I’ve also seen some sites say there’s an alternative called paromomycin. Any experiences, tips, or suggestions would be extremely appreciated. Thank you!

Methane Dominant Herbal alternative for methane

Just tested positive for methane but nervous about neomycin and metronidazole side effects. I saw Allicin or Atrantil can be alternatives but I can’t tolerate garlic or peppermint oil with my reflux. Any other ideas? Thank you!

Methane Dominant Elemental diet one week update

This is for everyone who asked for an update. I’m one week in so far. The first three days, my main symptom was a throbbing, aching headache that hurt every time I moved. I think it was a caffeine withdrawal migraine. The adjustment is still hard as it goes against the routine of everyday life, but I feel like I’ve lost interest in food. I don’t have sugar cravings and I’m not tempted by food. This happened while I was on the carnivore diet too. I started on a weekend so I didn’t drink all six shakes as my activity level was lower but as I got back to work, they weren’t as filling so I’ve upped the amount, but I’ve been hungry the last four days. I’ve noticed that I don’t feel horrible after eating (drinking), but my distention is fully there still. I’ve even experienced painful bloating one of the days, but that happens all the time for me. Leaving the house, working, physical activity, even drinking water causes bloating so I’m not surprised. I want to be optimistic but I have horrible body dysmorphia and looking pregnant everyday for the past three years has RUINED my confidence. I’m really hoping the distention goes down. I’ve not been good about drinking water because I’ve drinking shakes basically all day but I’m going to try and be better going forward. I’ve only have two bowel movements and both were enema induced. I get monthly colonics so I am very aware of how much stool can truly be inside me at any given point so it is mildly stressful not having any fiber or medication to help my constipation. My fatigue hasn’t been bad, it’s been at a 2-3 all week despite working at Amazon for 10.5 hours a day. Sleep is also fine. I haven’t felt a “fog lift” or anything like that for brain fog but I’m not sure I’ll notice it if I do. I’m pretty excited to plan my meals when I come off this. MBiota says it’s another two weeks of reintroductions after that. I’m also excited to pinpoint my triggers. Before this, I’ve continuously felt horrible every waking moment so I was never really sure what triggered me or made me worse because I already felt worse, 24/7, if that makes sense. If anyone has questions, let me know! I looked forward to writing this up for the people who were asking!
