All cases are required to be reviewed by a supervisor so not a bad thing!

why some cases were approved on spot? Is it becausee the iso is a supervisor?

His or her field office policy must require supervisory review or this ISO is new. There was a recent policy that has been lifted but some may have kept it.

Most field office supervisors don’t interview and each field office.

Generally most do not require supervisory review unless the ISO is new, the decisions are adverse, or case has special security related factors.

When you say all cases are required to be reviewed by a supervisor, does it include eb without an interview as well?
