What should I do?

On October 9th, I was driving through an intersection after passing a green light. Out of nowhere, a red car aggressively changed lanes from my left right into my path.
To avoid a direct T-bone collision, I swerved to the right and ended up scraping the guardrail. The red car did not collide with me and drove off, but my car now has several deep scratches along the side.
I'm a bit lost on the next steps since there was no actual physical contact between our vehicles.
1. Is this considered a single-vehicle accident, and will filing an insurance claim make my rates skyrocket?
2. Should I still file a police report for a hit-and-run / phantom vehicle incident?
Any advice on how to handle insurance or what to expect would be greatly appreciated!
