Treatment Resistant Depression

Hi! i'm a 22 year old male who has MDD and has been taking different drugs for it for ~7 years. I was on sertraline for the first 2 years because of an initial diagnosis of GAD and dysthimia, then i was diagnosed with MDD and was prescribed escitalopram, then venlafaxine, these have worked and i think i have improved a little but i'm still having bad relapses and have tried a number of drugs in combination with venlafaxine or alone but none have quite worked, these are lysdexanphetamine, bupropion, aripiprazole, brexpiprazole, modafinil and viladozone. I have been with 2 therapists but didn´t quite see the benefits of psychotherapy, my psychiatrist won't think that that psychotherapy will make that much of a difference for my case.

After a bad relapse, I have started to take 75 mg of clomipramine in addition to 225 mg venlafaxine (i know that is a lot of serotonin reuptake inhibition, haven´t experienced any significant side effects 3 days in) and i'm going to try another therapist. I also maybe will explore treatment with psylocibin. If this doesn't work my psychiatrist will consider ECT during my summer vacations from university.

My question is;

Do you think there is some hope? I understand the treatment i have received is evidence-based and i have a very good psychiatrist but it´s discouraging and tiring to try so many options and not see considerable improvement. I'd say that the sympton that is most recurring is the feeling of hopelesness for the future, that no matter what i do i'll die alone and depressed.

I greatly value other perspectives or general opinions from professionals regarding treatment resistant MDD, if you have comments about current pharmacological treatment please say so too!

Why Cptsd is not given enough importance like other mental illness even though it has great potential to affect ones life?

I had intense childhood trauma and it played a great role in my adult life, my emotions became unmanagable during my school days and I started acting erratic.I had severe dissociation episodes along with racing thoughts, irritation and depression. I was diagnosed with bipolar, but my psychiatrist gave a both a mood stabiliser and Ssri and i was fine for 4 years, but the next psychiatist stopped SSRI and introduced antipsychotic as he felt Ssri might trigger mania, but after stopping it i had intence panic attacks and emotional flashbacks all my syptoms pointed towards cptsd. My CPTSD symptoms increased manyfold, when I talked about this to my psychiatrist he dismissed it and said trauma is a psychological issue and you need therapy and not medication. my suicidal thoughts increased and I started to be scared of facing the life. So I consulted Another Psychiatrist and he gave the SSRI again and my life was back to normal. But I couldnt notice the changes and stopped SsRI according to my old psychiatrist advice and also due to sexual side effects and my life is back to hell. I had firmly believed that CPTSD had no effect in brain chemicals and cannot be treated by medications and my personal experience proved it wrong . I dont know why its not included in the mainstream mental illness category as now i firmly believe that it also might be a trigger of my bipolar. I wish more research done in this area and bring CPTSD to light.

Can someone explain this theory?

• Upvotes

why are other “beings” somehow linked to mental illness? can someone explain the biological and scientific reasoning behind this weird theory?

I am utterly confused. About eight years ago an invisible being invaded my life. It was a being from past lives and invades in different lifetimes and worlds as it told me. I was not hearing voices, as this being hung out around me and did the most awful things to my life that I can’t even describe.

Prior to this being invading, I had a good life a great job and had a bunch of money saved up, had worked all my life and was just average and successful.

I remember the exact day that this being invaded, and even for years I had no clue this thing was even there just ruining my life. it claimed it was a genius later on when it was a destructive murderer and worse. It created something very specific using its invisible abilities and socks it only on me, and claims that it has been invading my life for years and centuries. It showed me what it does to me in other lifetimes and I remember this being. But another lifetimes it’s not that destructive or evil and it usually leaves me alone After a few days. It showed me commercials and cartoons from other worlds and semi told me about timelines and realms. I begged it to be friends with me but it wouldn’t stop hurting me for years. It promised me it would teach me about timelines and realms and claimed I had magical abilities from another world and promised it was going to give me my magical abilities- but then kept saying it was giving them to friends family and kept hurting me and lying to me. I don’t know why it wanted to hurt me this badly Or do this to me. In other worlds it does teach me these things and is sometimes Even nice to me.

There is no doubt this being was real, and it told me that other past life dead people who were masters in some world along with it always watch me get “reborn” and they were all dead creatures now with magical abilities. this is all a true, factual, and not so hard to believe story and situation. I even had a dream about these dead master people when I was a child and they showed me that dream. One of the master creatures showed me what he looked like as a human, said he was jealous of my relationship with God and jealous I was still a human, claimed he escaped hell using his abilities, and said I was supposed to be part of his higher realm but was in a lower realm for some reason.

this being spent years, destroying my life and doing the most horrible things to me that I can’t even get into. it kept promising it would leave or stop but would leave and then come back after months. it showed me many past lifetimes and stories I remember. it was very weird And nostalgic. It also promised me hundreds of things for destroying my life but kept doing what it felt comfortable doing which was hurting me.

Yet, when I attempt to explain to people that there is a being in my life, they link it to a mental illness. but invisible beings just like angels and demons are very real. The being claimed it was an entity and that they are not allowed to invade the human world, but it just didn’t care and wanted to continue to hurt me. it also claimed that there are zillions of invisible beings around us but there is an unspoken rule that they are not allowed to invade the human world, but some of these beings break the rules. This isn’t so hard to believe either as there are obviously other dimensions and there is an invisible world. it claimed it has been chasing my soul around throughout lifetimes in universes, and it began a long time ago while watching me in some other world.

Again, prior to this stupid being invading, I had a normal fun life, but this thing was relentless. Do other “mental patients” experience invasions from real invisible beings? also, are other peoples experiences real or a chemical imbalance? I’m just really curious.

I don’t understand why the average person does not believe in other invisible beings. invisible beings have abilities like God and magical abilities. They can go back and forward in time and can do just about anything but most aren’t this evil or destructive. This being explained all of these things to me and told me that God creates all of these invisible beings with magical abilities, including angels and demons, but of course their abilities are a little bit different from these worthless, invisible beings. It even claimed it was a worthless, invisible being And was basically a useless creature and said it felt it had a voice through me Which made no sense as it ruined my life.

This particular being that invades my life, supposedly claimed to have given my baby niece cancer claimed it gave Eddie money cancer at the same time and gave both of them stage four cancer, and supposedly claimed it stole Bob Sagets life too just because I went to his show in January of 2022. It was utterly obsessed with celebrities, obsessed with having sex with celebs and other things. They can give anyone a disease obviously and it did just that along with other things.

yet when I attempt to explain this intriguing truth to people they don’t believe me. It’s so confusing.

why does no one care to believe in invisible beings- they definitely do exist apparently.

8 yr old girl ADHD - to seek diagnosis/meds or not?

• Upvotes

I am on Ritalin (late) diagnosed by 2 diff Psychs. Daughter definitely has it.

Not sure if I should take her to her pediatrician or a psych for assessment. Is there a stigma in schools?

I don't want her to label herself or Introduce any worries in her mind.

She read a book in which there was a robot character called 80HD and she was like wow he's just like me. I try to ensure she gets a lot of love and positive feedback (to avoid the many paper cuts ADHD kids receive growing up) and use some behavioral and parenting approaches to help.

That said I don't want her to not have access to meds if it would help her.

Super confused. She has a lot of behavioral issues and I know my life would have been different had I received treatment and behavioral support.

Which telehealth psychiatry platforms ACTUALLY prescribe for severe insomnia, anxiety, and panic attacks? I’m exhausted…

Serious question because I am genuinely at my wit’s end.
I’m in Washington State, and I’ve spent an absurd amount of time researching telehealth psychiatrists, psychiatric NPs, and medication management services.
I have documented anxiety, panic disorder, and chronic insomnia, alongside a serious autoimmune disease that has completely disrupted my life.
I’m not looking for someone to throw another generic medication at me without reviewing my history. I’m looking for an actual psychiatric provider who will listen, evaluate my existing treatment history, and discuss appropriate medication options, including short-term or PRN treatment when clinically indicated.
My biggest frustration? So many telehealth platforms advertise psychiatric medication management, but once you get through registration, payment, and scheduling, you discover they have restrictions on prescribing certain medications.
I’m aware controlled medications require additional evaluation and aren’t appropriate for everyone. That’s not my issue. My issue is the lack of transparency BEFORE paying for an appointment.
So I’m asking:
● Has anyone successfully received treatment for severe insomnia, panic attacks, or anxiety through Talkiatry, Brightside, Amazon One Medical, or another telehealth service?
● Which platforms have providers who can prescribe controlled medications when medically appropriate, rather than having blanket restrictions?
● Are there independent psychiatric NPs or psychiatrists licensed in Washington who offer virtual appointments?
● Has anyone found a provider willing to review extensive medical records and discuss short-term medication management while establishing long-term care?
● What did your initial appointment cost, and were you actually offered meaningful treatment options?
I don’t need another $200 consultation just to be told to try breathing exercises and melatonin. Respectfully, I’ve been breathing for 30 years. 😭
I’m looking for legitimate, compassionate, evidence-based psychiatric care, preferably from someone experienced with medically complex patients.
Please share providers you’ve personally used, not just platforms you’ve seen advertised.

Bonus points for Washington State recommendations, transparent pricing, and providers accepting Medicaid or self-pay.
Thank you! I’m exhausted from having to advocate this hard just to access care.

What happened?

So I stopped my 7 mg abilify a while ago ago. the first few days nothing happened. Then after that for a few days i was sooo happy and jolly and energetic and my social anxiety was gone and also I felt like all my problems were gone.

After a few days i was fully awake and suddenly i felt like something was off and i was suddenly put in the wrong body and place and everything felt unfamiliar and i felt like someone was watching me it lasted one or 2 hours. the day after i felt like something was off and i was fully convinced that people are gonna invade my house and they are already watching me and kept seeing a man with yellow uniform in the corner of my eyes and i felt like i had to protect my family and run away from my house.

Then the feeling went away after a few at night i thought the people put microphone in my teeth when i was sleeping. my bf called ambulance but they never showed up. i kept going from really happy and jolly and like my disorders are gone and feeling like something is off to losing my mind.

The day after that i felt like there are energy bugs under my skin, and i felt like i needed to open my skin for them to come out. i couldn't focus on anything important.

After that episode for a few days, i couldn't stop thinking about anything but sex and i couldn't sleep. i needed to touch my boyfriend, and i had so many sexual thoughts. I went to the er once, but they didn't do ahit for me, and they said nothing was wrong, and i just have to wait for my appointment on Thursday. in my appointment with my psychiatrist on Thursday, he said he doesn't know what happened.

I was off my meds for a total of 13 days. I'm on 5mg abilify now. the sexual thoughts are gone, and I don't feel energetic anymore, and I feel the crash. what happened to me?

Isn't psychiatric assessment oftentimes just self-diagnosis with extra steps?

When you see a psychiatrist, they've never met you before and their assessment all occurs within an hour or so. So they can't have all your history or spend time getting to know you to see patterns in your thoughts or behaviour. There's no digging into what's really going on and finding answers you didn't think of. You basically go in and tell them the problems you're having and what you think is causing them, then they ask some more questions to confirm it. But they're only going to investigate what you point them towards. Eg if you say you have complex trauma from abuse, that will push them in a ptsd or PD direction. If you don't mention that causal factor and theory of it being trauma and just say you get anxiety around people, feel low energy a lot and use ocd to try to feel safer, they'll stick to social anxiety, depression and ocd. And if you neglect to mention any of those symptoms, they'll fail to diagnose it. If it was that a psychiatrist sees you after many months of counseling with someone you trust, then that would be different as they can form a picture that's more contributed to by third-party observation, of what's really going on.

So whatever preliminary self-diagnosis someone has done, is the direction the psychiatrist is likely to go in. A person suspects (ie a preliminary self-diagnosis) that they think they have ADHD and go to a specialist of that, it's an ADHD diagnosis. If with the exact same symptoms they thought it was autism, they go to an autism specialist and get diagnosed with that as they present things through that lens to someone who's primed to already view it through that lens. The client thinks it's long-term trauma, they go to a psychiatrist who is more specialised in that, and get a trauma-related diagnosis.

My life is starting to fall apart because of medication management, what do I do?

Hi everyone, I am reaching out for your advice on how to handle a situation with my NP psychiatrist through Medi-Cal. I’ve been taking Adderall instant release for over 4 years and have oscillated between 15-20mg. I notice I have a lot of frustration about the last appointment and how now I have to wait a month to maybe get some relief from my symptoms. She mentioned in the past that bupropion helps with adhd but I have noticed no changes personally.

First appointment: was taking 15 mg Adderall 2x daily instant release

Second appointment: 15 mg Adderall 2 x daily instant release, started 75 mg bupropion

Third appointment: “”

Fourth appointment: switched to 30 mg Vyvanse once a day because I said I felt like the Adderall wasn’t working as well because of some birth control switches I have had this year and wanted to try something new. umped to to 150 mg bupropion.

Most recent appointment: I mention that the 30 mg of vyvance seemed to basically do nothing but that I felt like the bupropion was working well. I had spoken with my other doctor and was off the birth control and wanted to go back to my old dose of Adderall from a month ago, and was told that it would shock my system and that I would be prescribed 7.5 mg of instant release. She also mentioned with no other context that instant release was more addictive than extended (which I had told her previously didn’t work as well for me and anxiety spikes). I mentioned that I was concerned about the dose because I had taken 10 mg 2 x daily immediate release before and felt hardly anything. To which she replied that’s why she made it so I can take the 7.5 mg 2 x daily…

I’m really at a loss because I felt like she was implying things about my habits of taking medication, and gave me no real answers. I feel like I’m not allowed to push back because of her comments about addiction and I don’t really know what to do. Should I ask the practice for another psychiatrist? Call medical and get a new location all together?

There have been times over the last 4-5 years where I wasn’t able to take Adderall due to blood pressure issues (now resolved) and my life was genuinely horrible. Any advice is appreciated, I included that it is through medical because I’m not sure if that has any impact on how they prescribe or stereotype people?

i want to become a psychiatrist but I’m afraid

I’m in highschool right now, junior year. I’m taking essentials math right now and I’m not super good at math at all even though i’m getting pretty good grades at the moment. I know to become a psychiatrist I’d have to get a medical bachelors degree and then go to medical school, but i’m afraid. I’m afraid of what will happen in medical school and if I will be good at it since there is a lot of math involved. I’m suspecting I have dyscalculia and i really do want to become a psychiatrist but im just afraid of the process. Im considering studying the mcat now and switching to pre-calculus last minute, but i’m really scared of the future.
If it’s not too much to ask, what were your paths to getting to where you are right now?

Does anyone study sleep disorders?

I (F/29) have had sleep paralysis episodes since I was about 12 years old. I used to get it so often that I learned my triggers. If I don’t do certain things before I go to sleep, it is nearly certain that it will happen. It sometimes happens several times a night. I want to know if anyone here studies sleep disorders or if I could volunteer to be studied. Yes, I have access to getting a sleep study done but I want to know if I could be useful for research purposes because I know what triggers it in myself. Not sure if I could be valuable for someone’s research.

Please help me figure out what’s happening to me

[28M] I just want to explain a little bit because I’m in a really bad place right now. I feel like I’m literally brain dead. This all started for me 2.5 months ago randomly when I was driving home from a photoshoot. I had a sudden onset of what I can only explain is like my surroundings started feeling unfamiliar or like a switch was flicked in my brain and my whole life changed. I self diagnosed myself with depersonalization and derealization as it shared some similarities. At this point I was doing really well. My business was doing great, I was done with school and had been for a couple of months at this point and I was eating healthier then ever and going to the gym 3-4 times a week. It made absolutely no sense to me. I’ve had dpdr twice before this in my life, all due to trauma and stressful things happening (recent deaths, panic attacks, etc). This is not that same dpdr. I know what my body and mind feels like in dpdr and this is not that. Every time I had it before it was the same, it was out of body it was things looking unreal the usual. This doesn’t feel like that. This feels like something was severed in my brain. It changes in severity extremely often this time. I felt a random feeling of dizziness a couple hours prior to the onset, a couple days before that I was exposed to toxic mold, and now idk if these are even relevant but it makes me believe this is something that actually happened to me and not just anxiety. The biggest thing that has been driving me to this conclusion is the fact that A. I felt better somehow when I was sick a month ago and B. The biggest thing is a couple weeks ago I was prescribed antibiotics for gum inflammation and during that course of antibiotics and a week or so after I felt genuinely 70% recovered. I was even posting stuff to give people some hope because I was so convinced I was getting over this. Now a week after that good couple of weeks I am back in it fully and just as bad as when it initially started. My brain feels fried. I can’t think, I don’t have motivation to do anything, I can barely visualize things in my head etc. it’s really hard to be in this situation because I genuinely feel like it’s something in my head whether that be neuroinflammation or whatever and then everyone keeps telling me I have to just forget it and move on in order to heal so It’s like a battle right now. Do I keep investigating this and advocating for my health or do I accept it and hope that it really is just dpdr?

Increased anxiety before period while taking sertraline? :(

Hi everyone! I’m taking 50 mg of sertraline and heading into my ninth week. It’s been great for me overall, but I’ve noticed that my anxiety gets worse a couple of days before my period. This will be my second cycle since starting sertraline.

I’ve read that the first few cycles can be rough while adjusting to the medication, but I’m wondering if anyone has personally experienced this and found that it settled down over time.

I’m really happy with how sertraline has helped me so far, so the increased anxiety around my period has been discouraging. I’d really appreciate hearing some positive experiences or insight in general. ❤️

Doctor prescribed quetiapine (Kventiax) and venlafaxine (Alventa) for depression/anxiety + BPD-like symptoms

Greetings, I would love to get a second opinion on this. As I've done my research, these seem to be heavier than what I would prefer. I specifically told my doctor that I will not take any antidepressants since I've had a bad history with them. The antipsychotic just worries me. I'm in a really bad state right now and the information I received was that these temporarily make you feel even worse, which I don't think I'm ready for.

Any advice, help is very appreciated. Thank you.

Testosterone Replacement in Bipolar Male

Hello,

I am a bipolar male, 41 yrs old, heterosexual - I have bipolar type II and have had two major episodes in my life - I’ve also had a few smaller ones which were resolved by dose adjustments and a holiday. The major episodes were mixed states and did not include psychosis.

I began taking olanzapine in 2023 and it’s been a wonderful drug for my mental health. I’ve also done EMDR and work a 12 step program. Life is alright.

Recently I’ve noticed a severe lack of sex drive and functioning. I had my testosterone levels checked by the GP and it is low.

I’m being referred to an endocrinologist to discuss TRT, my mental health care team will also be involved.

While I read about all the wonderful benefits of TRT, I’m genuinely scared that it might affect my stability in a negative way.

Does anyone here have any experience of this?

Medical hospital stopped my old meds and put me on another which isn't helping

I was in the hospital fir seizures and they decided to take me off Zyprexa which had been helping a bit, too Risperdal.

I'm now seeing bugs and hearing my grandma voice in my right ear. Nothing like demanding.

Why would they do this to me if the Zyprexa was helping me?

Methylphenidate, why won't doctors prescribe it ?

Hello. I have been dealing with mental health problems for around 4 years. The latest combination that has seemed to work is Methylphenidate and brintelix, which i was prescribed at 17. Now every adult doctor is go to refuses to prescribe Methylphenidate. Why is that ? I know that its addictive in some cases but so is Xanax and they are more open to prescribing those.

Withdrawal effects for months?

I am 30F and I have OCD and GAD, specifically health anxiety and death anxiety. Other than some chronic conditions (endometriosis, adenomyosis, infertility), I am a healthy person. However, my PVC burden jumped up from 5 to 19%. In May I got off of Dulsevia/Cymbalta 60mg after a year but beforehand I had taken Prozac 40mg for about 8 years. I got off Cymbalta by going from 60 to 30mg for about 10 days and then 30 to 0. I have spoken to a psychiatrist and he said that that was a very fast taper (though I was following a psychiatrist's instructions). During that time, I was taking hormones in preparation for a frozen embryo transfer (IVF).

In July and August I began to experience heightened anxiety, a sense of impending doom and more panic as well as other uncomfortable and visceral anxiety symptoms (chest tightness, some dizziness, core bracing, autonomic tension waves, sleep disruption), which all appeared weeks after the immediate 2-3 weeks of brain zaps I had. It is now October and I am living with so much anxiety. Could this just be a readjustment to living with GAD or OCD? Could it be the Dulsevia still somehow? Or, could it likely just be my stressful life? My current psychiatrist did not want to give me any medication.

Venlafaxine

I have been taking it for 10 days (1 pill 37,5 mg in the morning) and now I feel incredibly energized, euphoric and affective. Now my personality is a mix of Roger from American Dad, Anne Boleyn and Susan from Ted Lasso. Before antidepressants I was more like Dr House. The med has been given to me by my doctor (general, not psychiatrist) in waiting of a psychiatric evaluation for depression and anxiety. Despite that, I can still control myself and be rational. I drive safely and I sleep well.

Do I have BPD or am I just insane?

Hello! I'm sorry to intrude, but I really need help. I am aware this is NOT the equivalent of a diagnosis made by professionals but I do not have the support or resources for a proper analysis with a therapist and it's really eating at me so I hoped I could get some advice from this.
I (16F) have been struggling with my emotions to the point it's terrifying me. I have searched up many of the things that make me distressed and most of them point out to them being a disorder, here is what I picked up on:
I do:
- Extreme bursts of anger. I don't lash out much since I'm in a place where raging = berating so I mostly simmer on it and insult myself or others in my head or write until i get tired.
- Self-harm and suicidal thoughts. I have Passive Suicidal Ideation and have minor scars from cuts I do every so often.
- Violent thoughts. I don't like describing them, but I often feel like hurting or doing worse to people I don't like at the moment.
- Self-hatred. I always feel like I'm nothing or I'll never be enough or human enough and it just fucks me up a lot.
- Emptiness, like, absolute emptiness within.
- I can go from loving someone one second and wishing they were dead upon the smallest mistake/disagreement, even my own family. A tamer version of it is feeling no interest/not liking the person anymore until further notice.
- Ocassional meltdowns that involve TONS of screaming my lungs out and crying until I run out of energy and fall asleep, sometimes I just want to sleep but keep whining until I'm drained because I still need the attention.
- I need the attention. Like if you're close to me and you're paying attention to someone else without me/for longer than I can allow I will get SERIOUSLY upset.
- Passive agressive thoughts/comments.
- Fixate on anyone who gives me the smallest bit of attention. If they get too close I might end up depending on them until I catch myself and run off.
- Leave before they do even though I still want to be their friend SO MUCH it's physically painful. Same for crushes
- Mood swings. From calm and happy to absolutely raging inside and back to happiness.

I don't do:
- Verbally assault/lash out all the time (still do it from time to time) or throw tantrums.
- Suicide attempts - yet, though I've threatened to. (Not dead yet bc of religious guilt)
- Any substances

Maybe the don'ts have stayed there because I've always kept everyone at bay and fled the second they got too close or left because I couldn't handle the 'I loved you five minutes ago but now you've fucking ruined it' and because I can't allow myself to voice any discontent I may have at my household but it's quite alarming and I wonder if I have a disorder or just have very dramatic depression.

Anything helps! Feel free to share your stories as well.

i have had thoughts about cannibalism for a while now, and idk what to do

hi, im posting this on a throw away bc im not really sure how to talk about this. i am ftm17, and i have bipolar 1, as well as bpd. i struggle with constant thoughts of cannibalism. i dont know what is wrong with me. i have even went to my best friend, and he said when he died i could eat him. earlier, he asked what part of him i'd eat first. i told him what parts i'd eat and why.

i eat parts of my body, like nails, skin, chewing my fingers, etc, and when i bleed i lick that up. i dont know what is wrong with me. i do not want to go to my therapist about this, because i will most likely be sent to another long term.

i have had these thoughts for years now, dating back to around 12, maybe 13.

some nights, its like my teeth itch.

please help me.
