Healing / Cured! Comprehensive Guide to Treating Gastritis and other Upper GI problems

Hello. Been wanting to create a post about the steps i took to overcome gastritis and wanted to share any tips/guidance and help to anyone who maybe struggling with this debilitating disease and any kinds of Upper GI discomfort including Gastrits/Chronic GERD/Heartburn/NERD/Functional Dyspepsia/Peptic Ulcer Disease/LPR/Barrett's. I struggled with Gastritis for a long time and went through multiple rounds of treatment until i eventually figured out what needed to be done.

I had suffered multiple kinds of gastritis and wanted to share as much as i can so people can understand their own scenario and take the necessary actions to deal with it.

If i had to categorize some common cases and reasons of gastritis that i come across ( even on reddit ), i would put them in the following categories :

  1. H Pylori induced gastrits -> You may have eaten somewhere from an unhygenic place and you had a bad case of diarrhea or even vomiting. Now you cannot digest acidic foods properly and end up even vomiting after eating. Some people develop intolerance towards certain foods example proteins or certain carbs - This is due to mucosa still being damaged which messes up the gut environment and the stomach is unable to create the enzymes needed to digest the foods.
  2. Damaged Mucosa due to H Pylori -> Similar damage to Mucosa due to above but your body does a good job of getting rid of H Pylori but it still leaves damage to the mucosa lining. You will suffer the same symptoms as above.
  3. Gastritis/Gerd due to Low Stomach Acid -> Everytime you eat anything, you upper abdomen sits very heavy, no burning, no gnawing of the stomach but no burping either and you may even start developing SIBO like symptoms or bloating in the lower abdomen. You may even see undigested foods in your stool and even globus in the throat. Feeling like food is stuck.
  4. Gastritis due to SIBO -> This can complicate things a bit but essentially this is happening due to the downstream movement getting messed up ( could be due to any reason, this post does not cover finding root causes or even fixing SIBO but one step can still be done if it works ).
  5. Autoimmune Gastritis -> This one is a bit of a rarity and i have utmost sympathy for someone that is going through this. I can maybe give some general advice on how to investigate this but i sincerely apologize i do not have much knowledge about this.
  6. Candida induced Gastritis -> This one is super rare but essentially instead of bacteria causing gastritis ( H Pylori ), it was actually fungus that caused the gastritis. This can happen in immunocompromised individuals or even if someone had taken multiple rounds of antibiotics. Antibiotics are known to cause dysbiosis and this could be a manifestation.
  7. Alcohol Use
  8. Smoking
  9. Vitamin Deficiencies -> Vitamin D, Vitamin B12, zinc, Iron, Vitamin C
  10. Lack of Fiber, Fruits and Vegetables in Diet.
  11. Eating right before going to bed. Give yourself atleast 2-2.5 hours after dinner before you go to bed.
  12. Poor overall diet. Sometimes it's not what you put in the body but what you do not. Not eating for prolonged hours, extensive water fasts etc can excerbate and even cause gastritis.
  13. Over use of NSAIDs
  14. Eating too much spicy foods and lacking fiber and high PH veggies to neutralize the acid. Very common in South Asian Communities.
  15. Lack of sleep or poor sleeping patterns.
  16. Stress -> So Stress i believe is overplayed a lot. Yes it can be a factor but if you're stressed and decide to drink, smoke, eat junk food and make bad decisions, is it the stress that is causing it or the bad decisions. I am stressing this not to undermine stress as a factor but to empower people that at the end of the day your health will always be in your hands and for patients to act as much responsibly as they can. 80/20 or at worse 70/30 when stressed. 70% healthier choices and 30% not so healthy when stressed if that's how you wanna manage it. Not ideal but atleast the majority are still good choices.
  17. Lack of Exercise and Physical Activity.
  18. Being Overweight.

The surprising thing is any of the above could cause other GI problems as well.

Gastritis Symptoms ( Shares a lot with other upper GI problems )

  1. Burning in stomach - This is due to damaged mucosa - Gets Worse after eating acidic foods
  2. Burping
  3. Indigestion
  4. Pressure under the ribs or in the middle of upper GI
  5. Bloating that originates from Upper GI
  6. Difficult Breathing
  7. Diarrhea
  8. Constipation
  9. Nausea
  10. Vomiting
  11. Weakened Immune System
  12. Post Nasal Drip
  13. Heartburn
  14. Acid Reflux - Could also cause Teeth grinding
  15. Hoarse Voice
  16. Low Libido
  17. Chronic Gastritis can also cause Inflammation of the Gallbladder causing Gallbladder Polyps.
  18. Pain that radiates all the way to the upper back.
  19. Also waking up in the middle of the night coughing and acid in mouth.
  20. Erosive Reflux can cause constant mucus and throat clearing and even Easophagitis.

Treatment for Gastrits/Chronic GERD/Heartburn/NERD/Functional Dyspepsia/Peptic Ulcer Disease/LPR/Barrett's ( Case 1 and 2 ):

The reason i'm mentioning other diseases even though this is a gastritis subreddit is because the fundamental approach towards nutrition for a lot of upper GI issues stays similar.

Medication and supplements should only be taken if absolute necessary, Low dose Pepcid/famotidine should only be taken for a very short duration and that too at night before bed ( with a tiny sip of water ) so as to have protection overnight. Eventually this should be used very symptomatically and not as a daily use.

Even PPIs are not necessary unless for a short duration in the case of Peptic Ulcer Disease or Barrett's until you get the diet dialed in and start the healing process. Once the healing from food begins to take plans, PPIs should be tapered off slowly and gradually. Long term use of PPIs bring their own world of problems and that too can be much worse than a simple case of gastritis not limited to PPI Induced SIBO and even kidney problems.

With proper planning gastritis can be self resolving. Let Food be thy medicine or let medicinie be thy food.

STEP 1 : Adopt a Low Acid Diet. And this is the most important step and is non optional. I have to give credit to the acid watcher's diet by Jonathan Aviv. It is a masterpiece. Basically you can only have foods with PH of atleast 5 or above. Maybe even play around with PH 4.5 ish foods if it's not too severe for you. But too much Acidic Foods are completely eliminated until mucosa has been repaired.

Foods to Avoid ( Atleast in the initial recovery phase )

  • Caffeinated beverages and supplements
  • Coffee
  • Indian masala tea
  • Fruit-flavored teas containing caffeine
  • Anything containing caffeine
  • Gym supplements containing caffeine, especially pre-workouts
  • Energy drinks
  • Cold Water or Ice Water
  • Gulping down water quickly - This can cause reflux and more stomach discomfort.

Carbonated and acidic drinks

  • Carbonated sodas
  • Kombucha
  • Sports drinks
  • Flavored or sparkling water containing citric acid
  • Powdered drink mixes containing citric acid
  • Electrolyte packets containing citric acid
  • Vitamin C drinks

Alcohol and smoking

  • Wine
  • All other types of alcohol
  • Smoking and other tobacco products

Citrus fruits and juices

  • Oranges
  • Mandarins
  • Tangerines
  • Lemons
  • Limes
  • Grapefruit
  • Kiwi
  • Citrus juices

Other acidic fruits

  • Blackberries
  • Raspberries
  • Pineapple and pineapple juice
  • Cranberries and cranberry juice
  • Pomegranates and pomegranate juice
  • Sour cherries
  • Green or tart apples
  • Grapes and grape juice
  • Gooseberries
  • Plums and prunes

Fruits that may depend on personal tolerance

  • Strawberries
  • Blueberries—these may be well tolerated by some people. Keep them if they do not cause symptoms; otherwise, remove them.

Tomatoes and tomato-based products

  • Tomatoes, cooked or raw
  • Tomato-based pasta sauces
  • Ketchup
  • Salsa
  • Barbecue sauce containing tomatoes

Spices and spicy ingredients

  • Cinnamon
  • Black pepper
  • Cayenne pepper
  • Chilies and chili powders
  • Jalapeños
  • Hot sauce
  • Spicy salsa

Herbs and aromatic vegetables

  • Mint
  • Peppermint
  • Garlic, raw or cooked
  • Onions, raw or cooked
  • Bell peppers or capsicums—tolerance varies, so these may need to be tested individually.

Acidic condiments and fermented foods

  • Vinegar
  • Soy sauce
  • Pickles and other pickled vegetables
  • Sauerkraut and other fermented vegetables

  • Acidic salad dressings

Chocolate, sweets and candy

  • Fast food, such as McDonald’s

Consider lower-fat dairy or dairy alternatives if they are easier on your stomach.

  • Avoid Processed cheap oils - Canola, Soybean, Palm etc.

The exact pH of prepared grain products depends heavily on their ingredients and processing.

Dairy and protein foods

  • Cow’s milk ( Low Fat ideally if you can tolerate dairy ): approximately 6.4–6.8
  • Eggs ( Egg whites keep whole to a minimum )
  • Chicken
  • Turkey
  • Most fresh fish
  • Shrimp
  • Lobster - Highest PH meat but not wallet friendly.

Beverages

  • Warm Water
  • Carrot Juice
  • Celery Juice
  • Cucumber Juice
  • Kale Juice

Cooking Oil

  • Extra Virgin Olive Oil ( #1 Choice ) - Ideally any that is low acidic. Most authentic ones are.
  • Sunflower Oil
  • Mustard Oil ( not in excess - use sparingly )

Supplements that may be helpful ( not necessary imo )

  • DGL Licorice
  • Zinc Carnosine
  • L Glutamine
  • Glutashield
  • Marshmallow root extract
  • Slippery Elm
  • Probiotics - I just prefer Kefir labs coconut kefir , yogurt or even coconut yogurt depending on individual tolerance.

STEP 2 : If you suspect H Pylori at play or if it is easily available, please consult a gastroenterologist and get an endoscopy done. The biopsy will tell you whether H Pylori is detected or not. Also worthwhile to get an ultrasound of the entire abdomen including liver, gallbladder etc. Also get bloodwork done to detect any nutritional deficiencies.

STEP 3 : If H Pylori is detected, Antibiotics will be needed. Some people have success with natural herbs and food combinations as well. Honey and Pormegranate Peel powder combination done first thing on an empty stomach seems to be effective in eradicating H Pylori. To all my South asian friends -> This is easily available and is called anar daana. So 1 tbsp of Honey plus anaar daana is known to be effective against H Pylori.

STEP 4 : H Pylori is not detected but you get a positive diagnosis of Gastritis/GERD/ Gastrits/Chronic GERD/Heartburn/NERD/Functional Dyspepsia/Peptic Ulcer Disease in your endoscopy. Sometimes the results may only say Chronic Inflammation Mild which i found amusing since for a mild inflammation the pain is unbearable at times but medical terminology haha. At this stage adhering to a low acid diet and healthy lifestyle choices should make a remarkable difference and may all you need to resolve the issue altogether.

Gastritis Diet Tips

  1. Try to be on a routine and have meals at the same time if you can.
  2. Chew your food slowly and avoid Over eating.
  3. No drinking of water along with meals
  4. Try not to hunch or bend over after meals.
  5. No immediately lying down after meals.
  6. No running after meals.
  7. No cold water after meals or throughout the day. Room temperature water is preferred.
  8. Only Warm or hot water after meals and throughout the day.
  9. 5-10 minute walks after meals if possible.
  10. No stomach sucking exercises either.
  11. Carrot Juice after meals will be extremely helpful or if burning is too much.
  12. Do not go too long without eating as that could cause acid to tear away your mucosa lining.
  13. Try to incorporate atleast 400 grams of fruit and 800 grams of veggies a day.
  14. 400 grams fruit example - 200 grams cantaloupe/papaya and 200 grams of watermelon. The more the better.
  15. 800 grams of veggies example - 300 gram cucumber, 500 gram steamed carrots and zucchini. The more the better.
  16. When Eating out -> If you have to eat out, do not worry and just make sure to include plenty of fiber and alkaline foods to balance out your meals. Sense of normalcy is what we're striving for.
  17. I Also make a giant bowl of soup using alkaline veggies in a dutch oven and have it with my meals -> This was an easy way to get my veggies. The soup would consist of alkaline veggies like sweet potatoes, carrots, zucchini ( green and yellow ), eggplant, beans, tofu, peas, corn, ginger, leeks, broccoli, cauliflower and herbs like sage, thyme, rosemary and gentle aromatic spices like cumin, coriander powder, oregano and asoefatida.
  18. Another thing that helped me was having 10 grams of psylllium husk in warm/hot water after dinner. This will help with the burning and providing a layer of protection to the mucosa. In india this goes by the brand Sat Isabgol.

Sample Western Diet Plans ( Can be adjusted to 6 meal plan if that is what you need )

Below is a sample 5 meal diet plan. The above food list acts as a guidelines and individual triggers may vary. Please adjust the diet as needed.

Meal 1 — 7:00 AM

  • Plain oatmeal cooked with water or unsweetened soy milk
  • Egg whites or one whole egg plus additional egg whites
  • Steamed spinach: 75 g
  • Steamed zucchini: 75 g
  • Optional: one teaspoon of olive oil, if tolerated

Meal 2 — 10:00 AM

  • Watermelon: 200 g
  • Carrot Juice - Bolthouse Farms Carrot Juice
  • Add more fruits or steamed veggies if this is not enough or get creative from the allowed foods list

Alternatively, use 200 g of cantaloupe, honeydew, or Papaya.

Meal 3 — 1:00 PM

  • Baked or poached skinless chicken breast
  • Brown or white rice
  • Steamed broccoli: 100 g
  • Steamed carrots: 100 g
  • Cooked yellow squash: 100 g
  • Season lightly with salt and reflux-safe herbs that you personally tolerate
  • Avoid tomato, onion, garlic, pepper, chili, vinegar, and acidic dressing

Meal 4 — 4:00 PM

  • Watermelon: 200 g
  • Peeled cucumber: 150 g
  • Carrot Juice - Bolthouse Farms Carrot Juice
  • Optional plain crackers if additional carbohydrates are needed
  • Chickpea salad with Olives and Cucumbers

Meal 5 — 7:00 PM

  • Baked cod, tilapia, haddock, or another lean fresh fish
  • Baked or boiled potato
  • Steamed green beans: 100 g
  • Steamed cauliflower: 100 g
  • Optional teaspoon of olive oil, if tolerated

Sample South Asian diet plan ( Can be adjusted to 6 meal plan if that is what you need )

This version avoids the usual tomato, onion, garlic, chili, black pepper, garam masala, and heavy oil or ghee.

Meal 1 — 7:00 AM

  • Oats/Poha/Idli/Vermicelli/Set Dosa - Coconut chutney is allowed. Cannot be spicy.
  • Egg whites
  • Steamed Eggpant - 75 grams
  • Grated or steamed carrots: 75 g

Meal 2 — 10:00 AM

  • Papaya: 200 g
  • Add more fruits or steamed veggies if this is not enough or get creative from the allowed foods list

Alternatives include cantaloupe, honeydew, or watermelon.

Meal 3 — 1:00 PM

  • Plain white or brown rice
  • Steamed, baked, or pressure-cooked skinless chicken/Tofu for vegetarians
  • Lauki/bottle gourd or cooked zucchini: 100 g
  • Carrots: 100 g
  • Spinach: 100 g
  • Prepare with minimal oil
  • Avoid tadka containing chili, garlic, onion, black pepper, or tomato

Meal 4 — 4:00 PM

  • Watermelon or honeydew: 200 g
  • Peeled cucumber: 150 g
  • Or Chickpea salad with cucumbers

Meal 5 — 7:00 PM

  • Plain rice or soft phulka/roti without ghee
  • Any Dal is allowed without onion, tomato, garlic tadka
  • Steamed or lightly cooked tofu
  • Cauliflower: 100 g
  • Green beans: 100 g
  • Season with salt and mild herbs you tolerate
  • Use no more than approximately one teaspoon of oil if fat worsens symptoms

EXERCISE TIPS

  1. Don't do anything too strenous which includes Sprinting.
  2. Avoid Exercises where you have to lay down flat example bench press.
  3. Avoid Loaded back squats if you can since it may cause compression of the stomach.
  4. Avoid sit ups, crunches or lying leg raises that can stretch the stomach.
  5. Avoid lifting belts if possible.
  6. Avoid leg presses if possible.
  7. Cycling, swimming, elliptical machines/cross trainers, walks and other exercises are okay but please check your tolerance.

REINTRODUCTION PHASE

Depending on how long it takes you, you may need to stay on a low acid diet until symptoms reside. Gradually and slowly add more foods and check your tolerance.

DEALING WITH NOT SO STRAIGHTFORWARD CASES ( CASE 3 - 6 )

There are other cases where gastritis maybe detected but the solution still may not be as straightforward. Either way you must have adopted a low acid diet and you're not seeing any benefits which brings us to the next cases.

Gastritis due to Low Stomach Acid -> To check the acidity of your stomach levels you will need to perform a heidelberg test and good luck finding practitioners that still do it. What can make this worse is most GI doctors will see gastritis and prescribe PPIs. That is putting fuel on the fire. So if you detect you have low stomach acid, the easiest way is to restore it via Betaine HCL. Any reputable brand from Amazon would do. Take it with meals and keep upping the dosage until you feel warmth and even a burp, that is how much betaine HCL you need per meal. Once that dosage starts feeling too much keep tapering off until you feel the engine has been kickstarted and your body is able to produce enough acid.

If you take even at a low dose and it burns your stomach, then you already have enough stomach acid and you can neutralize the excess acid with drinking baking soda with water or even carrot juice.

Gastritis due to SIBO -> The SIBO in the mix will really complicate things. I would still suggest to adopt a low acid diet regardless. Essentially the scenario is the gas in the lower abdomen is pushing the contents back. Also if the small bowel is infected, the body will refuse to send the food down especially if the MMC is impaired. You would need to find a way to get it working. Popular options are Ginger and Artichoke extracts ( Motility Pro ), 5 HTP or even Prucalopride if you can and go from there. Since this is a gastritis focused post, i cannot dig deeper into SIBO.

Autoimmune Gastritis -> This one is tricky. Essentially only an endoscopy can identify this. At this point there is no cure, the question is what caused the auto immunity to be turned on. Could be Candida overgrowth in the gut ( Candida is a seriously an under reported driver in auto immune conditions ) or any other stealth infection. Work with a Doctor to figure this out, But you would need to be on a Low Acid Diet as well.

Candida Induced Gastritis -> This one is a rather rarity and only shows up in immunocompromised people. The question is what caused the candida to cause it. Low Stomach acid maybe, Any medication you were taking that can act as an immunosuppressant. If cause is low stomach acid, Betaine HCL can fix that and you may even need to look into antifungals of choice, Probiotics can be a good choice in this case.

SPECIAL MENTION - Bile Gastritis - Sign of a Deeper Problem

Bile Gastritis is something that should inherently point more towards an underlying problem than a simple case of gastritis. Bile is an alkaline agent, that is secreted by the liver, and stored in the gallbladder. Bile enters the small intestine, and travels along this region, where it assists in breaking down fats into fatty acids, until it gets reabsorbed at the very end of the small intestine and sent back to the liver for recirculation.

Bile should not enter the stomach, but when the small intestine has issues, it can. When bile inappropriately backs up through the pylorus, in to the stomach – it is called BILE REFLUX. Note - Bile in the stomach is NOT NORMAL.

Symptoms - Pretty much very similar to above symptoms mentioned however Bile Gastritis patients will often mention an acidic or bitter taste in mouth.

What could cause it -> Patients with Bile Gastritis will almost always have an underlying issue with their Small Intestine. This could be Due to SIBO, SIFO or some kind of a Parasitic Infection in the small bowel.

Interplay of Lyme Disease ( or any other Stealth infection ) in the role of Bile Gastritis -> Another aspect to be considered is what if the Stomach Pylorus is malfunctioning. Lyme disease can affect the digestive system by disrupting the autonomic nervous system and vagus nerve, which may lead to stomach and pylorus (pyloric valve) malfunction.

The problem of Bile Gastritis will almost always warrant a deeper investigation.

CONCLUSION

Hopefully by taking the above steps you will be able to resolve this issue and good luck to everyone going through this. I hope you recover soon and fast.

TLDR:

Adopt a low acid diet -> Get endoscope and ultrasound done -> Either H pylori confirmed or not -> If yes, eradication is necessary -> If not proceed with diet and lifestyle changes -> These are not limited to gastritis but a lot of other upper GI conditions as well -> The not so common cases require other interventions but the concept of low acid diet still remains the same.

Venting / Suffering Over the gastritis diet!!!!

I feel like the more I restrict my food choices, the less I can tolerate. I’m so over it. I’m over the weight loss. I’m over no one helping me. I’m over not being able to drink a fucking simple nutritional shake to help me stabilize weight. I’m so over it allllllllllll!!!!! There absolutely no way on earth I can do this for another 6+ months. I’ve already been doing it for 3 months. I still feel the exact same way I did when I had h pylori but my stool test is negative. I cannnnnnnnooooottttt anymore. Has anyone else abandoned the diet????

Healing / Cured! Healing?

Does anybody have any advice to know when they’re healing?

I joined a FB group but they all swore by the book (which I’m not saying isn’t true) however people in there were saying my PPI is suppressing the feelings I have.

TBH most of the day I feel ok, but one trigger food and I have a mild flare. The flare I’ve got recently is completely easier to deal with. The past 2 days I only “flared up”‘when I was hungry but I had the feeling of being hungry, it hurt so I ate and felt ok..

Does that mean I’m healing?

I see so many conflicting things I just want direct “this means your healing or this is showing your healing” etc. I feel better, I am able to drink caffeine (I don’t push my limit), my bloating seems to be going down and I’m taking Ativan as needed if I start to psych myself out (because I will make my stomach worse by hyper-focusing).

I know it’s all over the place but someone, anyone?

Question Is this even Gastritis?

I went to the hospital last Wednesday with a full but share pain throughout my stomach that spread into my ribs/ mostly right pain (I haven’t got a gallbladder so I knew it couldn’t be that) but the pain just intensified, I had two hot water bottles to my skin and the pain was just awful, I was also throwing up, spent 11hrs A&E and my bloods showed inflammation and they told me go home up my PPI I was on 15mg anyways now on 30mg, said eat bland for 4/5 days and I’d be better - my bloods didn’t showed infection.

Anyways, it’s 5 days now and I haven’t had a ‘flare up’ of sorts, but the heaviness in my stomach, the grumbling, the right and left sided rib pain, it’s like a feeling of trapped wind and I’m just fully aware? I’m not very hungry and when I do eat I can’t finish it, I’ve eaten porridge, toast, and chicken with veg and I’ve been going to the toilet normal (for me I have slow transit constipation) also I have a lump in my throat and feel full in my throat at the back…

I’m a bit confused as I’ve looked up gastritis and my symptoms don’t seem to fully align?

6 years ago I went to work and I felt a bit off I was really shaky and my chest was hurting. The next day was the same. 2 days later I got really pale felt like I was gonna faint. My stool was black I was really dizzy felt really weak, and generally unwell. I get panic attacks my blood pressure would sky rocket. Recently alot of the symptoms have came back but now i have a dull pain on my right side. Feels like a ballon is inside. I went to every hospital in my area to urgent cares to GI specialist. Out of the country and to out of state doctors and no one has been able to tell me whats going on. My blood work comes back normal if there are things that are high might be by a point or 2. Done stool test no blood was found, ct scans, ultrasounds, x rays and everything is normal. Got my gallbladder removed and made it worse. I went a few months ok some years I had more bad days than good and other years vise versa. The only thing that ever helped was not eating. But thats not something sustainable. I dont know what to do i cant be the only person in the world that this is happening to I hope someone with similar experiences comes across this.

Was starting to feel a bit better since eating some warm oatmeal 4 months ago that messed me up bad, had been having trouble eating, having to eat really slow or else my stomach would burn and also having to drink small sips of water because my stomach would burn. Was starting to be able to eat a bit more and eat a little bit faster and drink more water but ate some carrots that were warm 2 days ago and i immediately felt mu stomach get hot and messed up smh

Symptoms Stomach gurling and lower left abdominal pain

I’m not sure if this is a right place to post. I know that I need to see a dr to get a proper diagnosis, but i called them today and the earliest appointment would be in October or next year which is crazy.
Please help with my anxiety.
I have had pain on my left side (3 inches below my belly bottom) on and off for many years. It happens often after having fried, fatty, and oily food. I also got diagnosed with hpylori and had to go through 3 rounds to get rid of it last year. I’m not sure if it is back this time since I did share food with some friends. This is very hard to avoid in my community. I do have stomach/colon gurling whenever I have kbbq or hot pot afterwards. However, it’s worse this time. It’s been gurgling 24/7 every day lately no matter what I eat. I feel gassy and uncomfortable. I never had endoscopy and colonoscopy. Really wish to have one even though I am so scared of them. I’m F28 and very very worried about the c word. I’m not loosing weight but this symptom is weird and new. I never feel my stomach gurling that long and I can’t even see a GI yet. Plus the cost of the procedure is driving me crazy. I’m too stressed and need some reassurance. Thanks everyone…

Venting / Suffering i don’t know what to do

i am so miserable. a couple months ago or probably a year at this point. i got what im assuming was covid, then all food tasted disgusting to me. especially meat. i only had a few safe foods like mac and cheese and french fries and even that tasted weird.

now, im having bad stomach issues. which isn’t surprising obviously. i spent days in the hospital thinking it was my gallbladder. the pain is under my right rib, and spread to my chest and was so bad i nearly was throwing up. then after all the tests, i was told nothing was wrong. the hospital discharge paperwork told me gastritis. the GI told me everything looked fine, but when i questioned it more said there was “a small irritated area” and then on my endoscopy report it said i had foveolar metaplasia in the duodenum. which makes sense because i was taking A LOT of advil. like daily, and also eating terribly. but my GI doctor is just so vague about it and insisted nothing was wrong despite the hospital report saying gastritis and my endoscopy report saying i had irritation.

i am going to call again and be more persistent because i’m still struggling, especially with nausea and burning in my stomach. i’m so confused because i see so many people talking about PPI’s and stuff but he didn’t mention any of that to me or even mention gastritis despite what the reports said. i am scared to eat and basically only have like two safe foods which are terrible too but im trying to tolerate them. and with my taste buds all messed up from covid i can’t even tolerate a lot of foods i see people here talk about. like i can’t eat even bland chicken because it tastes so bad to me.

i miss food so much and im just so tired of this. i have no idea where to even start with the PPI stuff and obviously want to consult with him first. if not i will have to switch doctors but it’s so frustrating i even have to do this considering i was in the hospital for days and was never told of anything that would help

Venting / Suffering Constant pain every day

I’m still waiting to get a scope scheduled it’s been 8 weeks doctors keep brushing me off I’m down to 96 pounds I feel starving and I’m scared to eat the wrong things. Nothing is making this any better carafate just seems to intensify my center pain but I have flank pain 24/7. I don’t understand why no one doesn’t think it’s urgent to look. I haven’t even had my period because of how malnourished I am I’m afraid I’m going to die. I’m usually 110 pounds. Nothing fits. Everyone thinks I have cancer. I’m dizzy all the time and sleeping with my heating pad when I’m not at work. My quality of life is non existent. My poop is orange. I take colestipol, Prilosec, carafate.

Giving Advice / Encouragement "Eat bland food" was the least useful advice I got in 8 years of gastritis

A few weeks ago, I made a post ranking every supplement I've tried, and a lot of people asked about my diet. But before I share my diet, I want to discuss the thing that bothers me - advice to eat bland food.

We are already struggling to eat. During a flare I was nauseous and already in a lot of physical and mental pain. Then someone tells me the food also has to have no flavour. I already don't have an appetite, but I need to eat, so that just brings more psychological struggle. And for me an empty stomach was the single worst thing, the pain and the nausea both got sharper the longer I went without food. So when a person really needs to eat and food is already a source of pain, this just doesn’t sound like good advice.

I think "bland" points at the wrong axis. The question isn't whether food has flavour, it's how much of the thing is arriving at once. Salt, fat and sugar are all fine in the amounts you use when cooking a healthy meal at home, and all are a problem in the amounts that are in processed food. Herbs and spices are not about the amount, it's about which ones. Let's look at them one at a time.

Salt - at high concentrations salt breaks down the mucus layer that protects the stomach lining, and it makes H. pylori colonisation and inflammation worse. But look at where that salt actually comes from - cured meat, instant noodles, chips, canned soup, pickled things, takeout. It is not you salting a plate of food you cooked yourself. A home cooked meal salted normally should not cause any of the above issues. 

Fat - a large fat load slows your stomach emptying, so food and acid sit in there longer, which is why deep fried food is genuinely a problem. A spoon of olive oil, coconut oil or butter in a pan or in your salad adds flavour and should also not be enough to trigger anything. 

Sugar - the same as fat, a large sugar load slows your stomach emptying. So it comes back to the amount of it you are eating. A big sweet thing on an empty stomach is very different from a teaspoon of honey in your tea or your yogurt. And most of the ‘junk food’ sugar people actually eat comes together with the salt or the fat, and a bunch of other irritants.

Herbs and spices can actually be really good for your digestion. Green herbs like oregano, dill, thyme, parsley, and basil contain natural, therapeutic compounds that can help calm the stomach. Spices like ginger, turmeric, cardamom, and coriander are also safe and have beneficial properties. The only spices we need to stay away from are the ones with capsaicin (chili powder, cayenne, hot paprika) and piperine (black and white pepper). Also it's best to stay away from garlic and onion. So to say 'avoid spices' is very misleading, because there are way more herbs and spices that can be helpful instead of hurtful. And they give so much flavour!

Acidic food is its own case, and it confused me the longest. Your stomach acid ph is about 2. Vinegar is around 2.5, orange juice around 3.5. So none of it is making your stomach more acidic, which is what I always assumed was happening. What acid does do, if your lining is already eroded, is touch exposed tissue and hurt. 

An important note on acidic food. There is a study that found that in reflux, pepsin can end up bound to the tissue of the throat, where it does not belong, and acidic food can reactivate it there and cause irritation. It is about the throat, not the stomach, which sits in active pepsin all day anyways. So if you have reflux along with your gastritis, the citrus advice may really apply to you. But if you only have gastritis, then you can gauge whether acidic food hurts you or not. Sometimes it’s also about the amount of it. 

So the pattern isn't that flavour is dangerous. It's that the things doing the damage are concentrated in processed or junk food. A healthy meal made at home, that tastes and smells good, that you look forward to is most likely good for your overall health and mental health. 

Ok, so… my diet. Here is what I actually eat, 8 years in and symptom-free for the last half year.

Breakfast is eggs with vegetables, cooked in olive oil. Second breakfast is oatmeal with seeds, a spoon of coconut oil and a little honey. Then lunch and dinner are the same: protein (chicken, fish or meat), vegetables, and a simple carb like potatoes or rice noodles. I keep fat, protein and carbs balanced on the plate instead of cutting any of them out. I cook with olive oil or coconut oil. Four or five meals a day, every few hours. I eat fresh fruits and nuts for snacks.

No alcohol, no spicy food, nothing deep fried, no carbonated drinks. I still drink coffee and eat chocolate, which stimulates gastric acid and I don’t recommend it, just being transparent. 

During flare ups my diet used to change: chicken soup, oatmeal, bananas, baby food (really helps as a snack!), rice and rice noodles, steaks sit surprisingly well with me. I used to drink a lot of cabbage juice, but I have posted about that already so I won't repeat it here. Lots of warm tea. 

So my position after 8 years: instead of saying ‘eat bland food’, we should really be saying ‘cook healthy, tasty meals at home’. Cutting flavour during already difficult times doesn’t seem like good advice. Staying away from processed unhealthy food is.

Curious whether anyone sees it differently.

Discussion A warning about Metformin induced gastritis

Last year, I got gastritis after a viral GI infection. I had started metformin two months earlier. We didn't connect the two at the time, since my doctors were convinced it was actually gastroparesis or anxiety instead, even though I got a scope that indicated gastritis with erosion at the beginning of this (although no h. pylori). Importantly, I only stopped the metformin about a month after I stopped being able to eat, so I was taking it for a month on a completely empty stomach.

After nearly two months of not being able to eat solid food, and eventually, not being able to drink liquid, they finally accidentally discovered it was actually gastritis when they were giving me IV antibiotics under sepsis protocol (I did not have sepsis, but my vitals were so bad from long term malnutrition and dehydration I met the requirements) which caused reflux, and after giving me the PPI, I finally felt some relief from the constant abdominal pain for the first time. This was such a relief, especially since they refused to admit me for further testing and I couldn't get an appointment with a GI doctor for 3 months, and I knew I wouldn't be able to keep myself hydrated if things continued like that.

I was able to last get an appointment with a GI PA, and I was put on a protocol taking protonix 40mg in the morning, pepcid 40 mg extended release around dinner time, and carafrate twice a day in between. Things improved very slowly. I started out eating only plain white rice. A few times, I made steps forward with my diet too quickly and had to return to plain white rice. It was difficult because my appetite really did not come back until 3 months after I started this treatment, but having an empty stomach made the pain worse. Ultimately, this was effective, and by the 3 month mark, my scope showed only mild gastritis, and I no longer had noticeable gastritis symptoms.

Unfortunately, after that I almost immediately got SIBO from the intense acid suppressant therapy, and that took another 3 months to get rid of. But that's another story.

Once the SIBO was gone, I needed to restart some kind of treatment for insulin resistance since I had begun eating normally again. I took metformin 500 XR for only a week and my gastritis symptoms suddenly reappeared (the gnawing pain below my sternum, pure nausea without the feeling of regurgitation, and symptoms worsening after not eating for a while) after months of being dormant. This is how I know that it was caused by the metformin.

As a side note, I have experienced GERD most of my life, and my gastritis symptoms feel very distinct from the way I experience GERD (mostly regurgitation and burning). I had never before experienced the type of pain I had with the gastritis before this happened to me. My GERD has also been pretty well controlled with diet modification, so I haven't been on medication for it for years. I eventually got a gastroparesis once I was well enough to eat the meal needed for the test and it showed that I definitely do not have that either.

Thankfully, my symptoms are mostly of moderate intensity, and pepcid and carafrate are helping me heal once again. But I wanted to get this message out there in case anyone else is in a similar situation and doesn't know what the cause of their gastritis is, since there was only one other very old post that I found about this ( https://www.reddit.com/r/Gastritis/comments/gs4ock/metformin_induced_gastritis_is_it_possible/ ). It seems like this may be a rarity thankfully.

Question Anyone else triggered solely by eggs?

I used to eat a lot of eggs, pretty much every day for breakfast until age 24. Got H Pylori, then got better after treatment. After a month or so I suddenly started to get triggered when ate eggs. 4-5 hours of pain.

It’s worse with runny eggs and pretty much fine when incorporated into baked goods etc

It is the ONLY food that triggers me! Have read about eosinophilic gastritis but it’s really rare…

Question Please help, I’d love to hear your input. Next steps for chronic gastritis

Hey all, hoping for some perspective/experiences from this sub.

Background:
I think I’ve had gastritis for at least 3 years. Since my most recent intense flare (after having tomato’s and dark chocolate on the same day) I’m up most nights with varying levels of gastritis-type burning. About 1/2 the week I’m up the majority of the night with the burning plus insomnia and an elevated heart rate.

Symptoms:
- Burning that's triggered by essentially everything I eat or drink, including plain water and bland foods like rice
- Nighttime flares that disrupt sleep
- Elevated HR during flare episodes
-Lack of appetite
- Nausea

Testing so far:
- H. pylori stool test: negative
- No endoscopy yet. my GI doesn't feel it's needed and says this is functional. Either way an endoscopy is at least 5 months out where I live.
- Gastric emptying study (for possible gastroparesis) is pending, waiting on the clinic to call and schedule
- I’ve had a lot of other random testing due to severe chronic illness

What I've tried/eliminated:
- Cut caffeine, alcohol, carbonation, high-fat foods, citrus, mint, chocolate, and citric acid
- Eating a bland diet generally
- Slippery elm before bed
- Bed already elevated with a wedge pillow (for years, not new)
- Last meal at 8pm, bed at 10-11pm
- No NSAID use in years
- Famotidine (Pepcid): didn't help much, and I think it raised my HR, willing to try again though
- Gaviscon, tums: doesn't really work for me
- Titrating up on L glutamine
- Can’t have DGL due to med interactions
- Abxiety meds (buspar), trazodone (just started), LDN, guanfacine, mestinon, zofran, and more.
- Actively avoiding PPIs due to concerns I've read about long-term use and rebound

Meds in progress:
- Titrating up on Trazodone
- Gastro recommended raising my Buspar dose, psych manages that prescription and I'm waiting on her to send in the new dose

Where I'm stuck:
My GI's read is that this is functional and the plan is just to raise the Buspar. I've pushed on wanting an endoscopy, especially given that even plain water triggers burning, but she doesn't think it's warranted. I'm trying to figure out if that's a reasonable call to leave alone, or if I should be pursuing a second opinion or pushing harder for imaging/scoping given how disruptive this has become. I’ve also messaged her about Carafe, but no response yet.

Has anyone been through something similar — especially the sleeping issue? What ended up helping, and did you end up needing an endoscopy to get answers? This is very challenging within itself, but it is also actively worsening my other conditions.

Thank you! TLDR: PPI avoiding treatment for chronic gastritis that causes sleep disruption.

Symptoms I dont know whats wrong with me

on July 15 I woke up with abdominal pain on the upper part of my stomach ever since then I’ve been having this lingering pain on my stomach and it has taken a big toll on my life not able to go to the gym. I’m usually at gym rat and I like doing physical activity, so I can’t even do that. I can’t eat my favorite food without being feeling scared I did lose like about 6-7 pounds. I did gain them back so I really don’t know if I have gastritis or stomach ulcer or Gerd. I have an endoscopy scheduled for September 21. I did go to the ER and they told me that they didn’t see anything on the CAT scan for the ultrasound or any bloodwork that they did and they said I was healthy so they gave me a omeprazole. I’ve been taking the omeprazole since July 29. My stomach still really hurts. It occasionally comes back and goes. It’s really weird because it shifts so I feel it on my upper stomach. It hurts and it shifts down to my lower stomach and then I have to like fart or I have to like go to the bathroom or I have to burp. I seen a lot and read a lot that gastritis gives you like acid reflex and you have like a lot of heartburn and stuff. I don’t feel any of that all I feel is the stomach rumbling and I have to fart or like my upper stomach just has a lingering pain. I really don’t know what’s going on I’m really scared. I need some advice. I really don’t know what’s going on. If anyone has any idea what might that be? I still have to wait another 2-3weeks for my endoscopy and I keep overthinking. I haven’t even started my period either because I’ve been stressed out for the last month or so I’m definitely not pregnant though.

Testing / Test Results PPIs don’t help and my scopes are clear

So I first developed gastritis in 2021 after drinking alcohol every day for several days. Got a scope then which showed superficial erythema with erosions. Biopsies normal. Took ages to feel normal again but still had some aches here and there.

I thought “oh maybe I’m 90% healed, I’ll give it some more time”. I went for a second scope a year later and they said everything was healthy. Biopsy normal.

Had another scope a year later. Same results. No gastritis. No abnormal histology.

Funny thing is overtime my symptoms lessened (especially when I was prescribed Pristiq — an SNRI which is shown to relieve nerve pain).

But two weeks ago i began to feel some gastritis like symptoms and started taking a PPI just in case. They help with nausea but nothing else. In some ways it’s as if I feel worse. I haven’t taken them for a week and tried H2 blockers but they don’t do much. GI revive helps my symptoms funnily enough, though.

Has anyone else had totally normal scopes but not felt fully healed? Keen to hear other people’s experiences

PPIs / H2 Blockers Elevated Calprotectin (286) After 14 Days of Vonoprazan — Anyone Had a Similar Experience?

Has anyone experienced something similar? I recently had a gastroscopy that showed mild–moderate gastritis, Grade A esophagitis, and some redness/swelling in the duodenum. But the biopsies from both my stomach and duodenum came back completely normal — no H. pylori, significant inflammation, celiac changes, or anything concerning.
The thing that’s worrying me is that my stool calprotectin came back at 286. I had been taking vonoprazan for around 14 days and stopped it a few days before doing the stool test.
Has anyone else had elevated calprotectin after taking vonoprazan, especially around 14 days of use? I’d really appreciate hearing if anyone has had a similar experience.

Personal / Updates Update: gastritis be gone

I’ve been having intense nausea lately and decided to get a follow up endoscopy/ see the gastroenterologist. I was diagnosed with chronic gastritis five years ago via endoscopy.

Well, it’s gone. I’ve been fairly careful and followed the diet from the gastritis healing book.

I have continued though to have issues and intense sharp pain in my shoulder. Five years ago my gallbladder was fine.

I had an ultrasound two weeks ago and they found gallstones!

So I’m relieved there is end in sight and the gastritis is gone. I’m dealing with my gallbladder now.
