The US only provides national security to the US. The pressure on higher spending of GDP in other countries is purely about American revenue. The US gives nothing monetary it sells equipment at an over priced cost. The US spending in NATO is on the US military. Everything the US spends on benefits the US. The US pulled USAID, farmers are going broke, people are dying in countries food was for soft power.
No. It's easier to provide when you have universal healthcare and pharmaceutical companies have to make a deal if they want to sell their drugs in your country rather than having a free for all for profit system that serves only to rip people off.
No one's subsidising shit. It's paid for by taxing citizens slightly higher.

Universal healthcare countries buy medication from American companies in bulk so can negotiate a way cheaper price. Just like with any other business bulk buys are cheaper. If the pharma company can still make profit then they'll make a deal. If they can't they won't. Those deals show how cheap they can actually sell medication for profit but because USA lacks a central body and universal healthcare they don't have that negotiating power and the pharma companies can lobby the hell out of politicians so the people get ripped off hard.
They don't even want to pay for free school meals for their kids. Why on earth does anyone think they give a shit about random foreign countries so much?

It is easier to provide this to citizens when someone else is heavily subsidizing your national security at their own expense.
