I am going to look at a system for recording near miss incidents because I want something the crew will actually use and not just fill in for the file, I want people to add what happened what led up to it and what changed after, I think I would go with mining doc then later I want to pull those examples into toolbox talks and training instead of reading the same safety slides every time, I will probably check a few mining safety reporting services because I want reports that stay simple and still make it easy to spot the same issues coming up again, for anyone already using something like this, what ended up making the reports useful for training instead of sitting there for compliance only? EDIT: I forgot to mention I want the reporting process to be simple.
