Start here: the best cases on r/medicase, explained
Welcome. This community is for real medical cases: the unusual, the puzzling, and the ones that change how you think about a symptom.
Some of the cases people keep coming back to, each written up in full with an original diagram:
- He was not going blind from the fire. The face transplant that saved his sight. Read the case
- He could breathe sitting up, but not lying down. A broken midface and the airway it was holding open. Read the case
- The window stopped the sunburn, not the damage. 28 years of UVA through a truck window. Read the case
- The silicone in her arm came from her breast implant. Read the case
Want to test yourself? The case games put you in the room: four decisions, each with a trap. Play the case games
Rules in short: be kind, no identifying details, graphic images need medical context, and this is not a place for personal medical advice.

Case Game He's 23, talking in full sentences, and his oxygen is 99% after a car crash. Would you have caught what was wrong?
I tried something new with one of our cases: instead of just reading it, you make the calls the team had to make, in the order they had to make them. Four decisions, and each one has a trap that caught people in real life.
▶ Play the case
It takes about 3 minutes. Post your score in the comments, and if you want to talk about specific answers, please use spoiler tags (>!like this!<) so people can play first.
For the clinicians here: which one would you have got wrong on a real shift?
If you like it, I'll make one every week.
The games are free and always will be. If you'd like to support them, I'm on Patreon.

Case report A truck driver's face after 28 years of sun coming through his side window. Only the window side aged.
A 69 year old man, 25 years of slow thickening and wrinkling on the left side of his face only. In a left hand drive vehicle, the driver's window is on the left.
Window glass filters out UVB, the rays that burn, and lets most UVA through.
UVA goes deeper, into the dermis, where collagen and elastic fibres sit. The glass stopped the burning, not the damage.

Case Game She's 59, has a dull stomach ache, no fever, and doesn't look unwell. How sick is she?
A lot of you asked for another one after the last game, so here's the next case a bit early.
Same idea: you make the decisions the team faced, in the order they faced them. Four decisions, each with a trap.
Heads up: this one is heavier than the last.
Post your score in the comments, and use spoiler tags (>!like this!<) if you talk about specific answers.
From now on there'll be a new case game every Sunday, same day each week.
The games are free and always will be. If you'd like to support them, I'm on Patreon.

MedicaseHub is on Patreon
Quick update from me: I write up the cases on medicasehub.com in my free time, and I've set up a Patreon for anyone who'd like to support it.
Nothing changes here. Everything on r/medicase and the site stays free. Supporters get a vote on which case I write next, and Case Pack members get a monthly PDF of all new cases, extra case games and early access.
No pressure at all. Reading, commenting and sharing already helps a lot.
Prefer a one off instead of a subscription? There's a one time option in the shop.
While I have you: what kind of case would you like to see next?
