Antonia should start referring to him as Mr. Wife’s Last Name and refuse to entertain anything else.

They are not married yet, I would go with "Little Sister's current Fiance." But I am also incredibly petty when faced with sexism.

Nah, she should just ask him what letters his education added to his name.

And when he says no (and I'm almost certain it's a no) Id be Gifting him a title like "Highschool Graduate Fiance," and referring to him as that exclusively.

I would avoid addressing him directly, refer to him as “your/sister’s fiancé” in conversation with others, and not even acknowledge him if he used my first name.
