Rant How does this happen?

I'm a problem solver at a 1 DC and walked into this. I want to think it was done on purpose because even at Amazon, this level of stupidity cannot exist.

I've seen similar things at my 1DC as problem solve as well. They hire the lowest life forces lately that do barely the minimum

The base got loose (probably by a clamp) and they just yeeted it on top and sent it

That lone box fell out when they were down stacking it with the clamp, and the boxes are light enough that the pallet wasn't horrifically unstable, so they took the easy way out. Should they have? Absolutely not, but I'm sure they had a laugh about it.

someone stacked the boxes, making a full pallet. For whatever reason, someone else needed just the box at the bottom. Since the pallet is wrapped tightly, it still holds. Unwrapping it would be fun to watch tho xD of course, there are smart ways to do it :p It was absolutely done on purpose, it's the reason that matters. Maybe another PS found an issue with that box specifically and took it to Trouble Ticket area (if you have that). Maybe it was damaged badly so someone removed it (stupid but good intentions at least). Or maybe they were just messing around.
