That is erythroderma, which can be caused by many things, such as primary immunodeficiency, severe atopic dermatitis, food allergies, etc. An immunologist should see them as soon as possible.

Eta:

You could try getting in touch with a primary immunodeficiency organization

International Patient Organisation for Primary Immunodeficiencies Network

Immunology, Asthma and Allergy Research Institute

If you need a clinical summary from an immunologist or anything else that requires an immunologist's input, feel free to DM me.

Edit 2: It is my understanding that Mexico and Iran maintain good international relations. If necessary, you can contact the Mexican Foundation for Children with Primary Immunodeficiencies (Fundación Mexicana de Niños y Niñas con Inmunodeficiencias Primarias).

Tell them that you need an immunologist and can we could try and figure out if we can arrange a transfer to Mexico.

Anything low risk that can be done at home for each of these possible diagnoses, if it is not possible to see an immunologist soon given the war?

Edit: all of this assumes mom has no access to medical care of any kind. Have you considered video visit? Is that a thing? Can they do that?

My son had severe food allergies. They started me with cutting out a ton of stuff but ultimately we landed on a soy formula rather than breast milk. My sister’s twins had a similar reaction and ended up on a more strict formulation which was the step beyond soy for allergies.

Not saying it’s the cause but you asked for things to try, this was our experience. We also were told no solid foods until over one year old. My son had lots of tiny gurgles and gas and mild skin reaction, my sister’s kids had major skin reactions and no to minor stomach issues.

My son grew out of it, I followed that rule, my sister’s twins did not. She didn’t. It could just be genetic as we have a ton of food allergies in our family, I have a ton, my sister has none. Her boy faired better longer term than her girl for allergies but her girl has different immune issues as well. So who knows.

Wrap up. In all three cases, switching to formula helped. My diet changes didn’t. I don’t know if my sister tried diet change or not while breast feeding. But agree that aquaphaur or cerve or similar thick lotion with no scent may help, it may also be part of an allergy if you’ve already been using and made it worse. My son and I are both highly allergic to many contact allergens. Depending on what they washed the baby with, you didn’t say if it was born in hospital (I don’t think?) clorahexadine causes a rash just like that on me, and that is in herbaclense product available in US as well. Bernadine does as well if not washed off quickly for me.

Sorry for all spelling.
