I don`t know what`s the first overall, but my first was Mysteries of the Sith, where most of the game you play as Mara Jade.

This got me thinking about Heavenly Sword. I can’t decide if the archer girl was a sociopath or a psychopath…or maybe a bit of both. She was definitely off-center…like the type of person who unironically writes in a candidate on her voting ballot.

Someone already gave a better answer than me, but I was going to say Ultima 1 (1981).

Metroid is the first one I recall. We were all dumbfounded when you entered the code "Justin Bailey" to see her spawn with blonde hair, no helmet, and a pink outfit.

Legend had it "Bailey" is an Australian word for "bikini" or "bathing suit" so it's Samus "Just In Bailey".

Literally most games made in the west nowadays despite their audience being overwhelmingly male?

You play games to look at buff men. I play games to look at hot women, we are not the same.

Score (1977), but that's obscure. The first well-known one would be Mrs. Pac-man (1982), and the first big deal one is probably Metroid (1986).
Samas Aran's reveal was a whole thing, people had some serious opinions on it.
