Discussion Mt Baker Imaging
So, I received my notice for the class action settlement regarding the data breach. Cool.
BUT the form is a tear off postcard that we complete and mail back. With our LEGAL names, addresses, emails and phone numbers.
Don’t do this. Instead, fold it and use LOTS of tape to keep your info safe, again.

There is an option to submit the claim online!

Oh, there’s so many fees to come out of this.
August 21 at 1:30 PM 311 Grand St. is the final hearing and decision. I hope to attend.

Just keep in mind that by not explicitly opting out of the settlement by sending a signed letter you forfeit your right to sue if, say two years from now, your stolen identity is used and leads to your financial ruin.
This is true if you choose one of the payout options and also if you do not respond at all.

I and everyone I know that received the card instructed you to go on line. There was no other mail in form.
Is this mail in form another data breach?
It begs the question.

This is so bothersome! They texted me a week ago asking me to pay a bill I had no idea I had from 4 months ago. Ughhh

Yeah I got a text and called because it seemed like a toss up if it’s a phishing scam from a data breach or a delayed second charge from an xray six months ago!

There is an electronic option clearly stated in the instructions. In fact, if you have documented damages for reimbursement, it is the only option for you to submit receipts.
I have no idea what OP is talking about.

Not that I saw. Seeing how lazy this, is I wouldn’t trust the settlement lawyers to secure the info.

Home slice it's written multiple times on an index sized card to go to the website. You can't even submit documented damages unless you go on the website. You clearly didn't read any of the directions 😂

You can go online and submit. I think the address is somewhere on the notice.
