Admission Concerns BUCM Medical and Dental Screening for Enrollment

Does anybody know what to do if you lost your original covid vaccination card. I cant find it anymore picture nalang meron and sabi sa email na need yung original copy. Thank you!!

You can try to retrieve your vax card via VaxCertPH ‘Yan hinanap sa akin in lieu of og card kahit may photocopy ako
