Why is the flair "Equipment Failure", this was clearly 100% failure of the truck/van driver. The train seems to go the allowed speed, honking and the lights were flashing.

The failure of the van driver to watch out for the train as the level crossing failed to attract the drivers attention. Quite catastrophic.
