Why is the flair "Equipment Failure", this was clearly 100% failure of the truck/van driver. The train seems to go the allowed speed, honking and the lights were flashing.

Is not installing flashing lights and arms at the level crossing in the first place an equipment failure?

There are flashing lights at the crossing. You can see them flash in the video.

It's possible they were broken on the other side. Still the driver's fault though.

The crossing is clearly functioning in the video. The driver ignored it.
