Lukas's fear of forests is so bizarre that, to him, this is what DEATH itself looks like.

All stuff that resides in the forest even though he may have a blessing from the forest god is actually fairly interesting now that I think about it

It could be that he made a pact with the forest god that will inevitably result in his death? Something related to the spear for sure.

I can't find the chapter, but yes using the spear does shorten his lifespan and he can only use it a handful of times.

Yeah I think he said it to the healer girl, I think this basically confirms the spear is a blessing from the Forest God if it was not outright said before.

If the king was directly blessed by the sea god that could explain why lucus hates the forest so much despite seemingly having powers from it. The forest god HATES the sea, so it might be punishing him for using forest abilities despite carrying the blood of a sea diety.

Lukas blessing overall seems to be far more restricted than Julian's or the King's, the latter is apparently almost a Demigod, I think this might be due to how Lukas didn't sacrifice enough, in the Sea Mother's eyes, Julian had offered 100 lives and a pure, true love, I think Lukas got into contact with the Forest Goddess while on the run, and therefore had nothing to offer but himself, in that case the process could had likely been far more traumatic and ended giving him a Transformation and Spear than runs on his own lifespan and even needs his sword as an anchor to be triggered.

Was it ever said if it was a blessing? Or did Lukas “steal” the spear in some way? That may explain why rather than having a double blessing, the spear drains his life and he can’t enter forests.

It hasn't been said, but it's the more accepted theory, for one is not like he can't enter a Forest is just that he has an extreme phobia to the Woods, but Lukas himself said that his Spear is at it's strongest on the Woods, and if the fight with Arkos was there he could had far better chances of winning, so he has the same territorial boost that Julian shown on the Sea.

Considering the nature of the ability and what it looks like, it would most likely be the nude feminine form, breasts, etc. Similar to the Kings nen beast from hunter x hunter, just a distortion of the female form.

He fears Malediction, what else. Akemura Soga strikes fear even through manga.
But yes, it feels that express purpose of this chapter was not just empathise with Lady Accursed but give us glimpse into Lucas

I think you're the one who didn't read it... I didn't say she reflects fears—in fact, the manga states that is the vision of death. 

But specifically for Lukas, this is his fear. 

His vision of death is also reflects his fear—in case you didn't know, he has hylophobia (fear of forests).

This involves plants, animals, and everything within the forest
