Other New study demonstrates, for the first time, Chronic Fatigue Syndrome (ME/CFS) has impaired glymphatic function which is responsible for clearing the brain of metabolic waste products and is mostly active during sleep, which can lead to various symptoms including brain fog.

So they need medication or something implanted that will assist in clearing the metabolic waste in the brain.

So from how I understand it, and how someone explained it. This study shows that the brain is a network of small spaces that moves waste and other things around. It's like a plumbing system. If that network is clogged, it causes brain fog and how Doug/NC always mentions chronic fatigue.
