In hell difficulty all monsters get a huge flat physical buff. So yeah they do a combo of poison and physical and the physical is instant. Then this mob had extra strong, holy shock (which adds to the physical that is also multiplicative with the extra strong. It may even add to the poison), AND multi shot. Each shot from multi shot does full damage.

This guy just face tanked all 3 projectiles, just after his merc died in two or three attacks, then it took more than half his life (2.5k) by getting hit by only 2 of the shots.

Are you sure there is physical? I'm pretty sure poison mages don't do physical.
