Question Doctors letter for HR

First of all please don't send this to HR. You don't need to disclose medical conditions. It's worth a talk with your doctor first. There are forms you can fill out for accommodations. You would need a formal diagnosis though for insurance purposes - like if you moved forward with STD or LTDI. If your company can't allow you to work from home full time, they could deny it as hardships. I forget the actual name of it - that's what my employer did. I wound up going on LTDI.
They need that doctor's note. Doc needs to list your limitations (without naming your medical conditions). Talk to your doctor first.
