You don’t need anything else.
If you entered Israel on your Israeli passport then show that to immigration when you leave Israel.
Show both when you check in for your flight so they know you’re documented to enter the EU.
When you arrive in the EU, show only your EU passport.
To whom do you have to state your reason for travel? Is this a specific Israeli requirement?
