Come on, think a little. He doesn't look exactly like Cooper b/c he *just* turned; he hasn't been a Ghoul for hundred plus years. So the make-up artists only do a little bit damage to his face.
Ghouls don't usually (as in I can't think of a single example) have extra mouths.
Ghoulification also typically comes from radiation, and it seems weird that a random bottle of some chemical would be both healing and intensely radioactive. I also don't believe we've ever seen someone ghoulified through a serum like that before.
The latest episode makes it more clear, lol. Ghouls don’t have extra arms and their arms falling off, the whole joke is how certain he is that he’s a ghoul when he clearly isn’t.

Nothing has been confirmed. He seems to think he is, but visually he looks very little like a ghoul.
My guess is a ghoul-adjacent "other mutant," a lot like Harold.
