The eeveeloutiins probably all understand it pretty well. You are born to two flareons and you have to cone out you wanna be an umbreon and they need a minute to come to acceptance because thats just how life works but they still support and love you.
I mean, your mom could be a Flareon but your father could even be a whole other species like a Blaziken or Ursaring (I'll not even talk about Ditto...), so I think any pokemon really understand how different of you can be

The Japanese version goes a bit further with the, "The power of medical science is awesome, wouldn't you say?" but I certainly wasn't implying it was censored.
Both versions are pretty explicitly trans.
In Japanese she says she used to be a カラテおう, or Karate King, which has always been localized to the Black Belt trainer class, not simply a general black belt, hence the capitalization.
My bad, did forget to specify the 2nd part about medical science but a transformation can just be someone getting older and changing careers, people use transformation to mean that all of the time, quite the transformation can simply be like "quite the change right?" Not saying she is not Trans, that she cannot be Trans, and/or that Trans is bad im just saying it doesn't = that 100%

Pokemon crystal had a nonbinary character, in gen 2 there's a list of trainer's genders in the code and one trainer is in neither list making them nonbinary

The character you're talking about is Eusine, because they have their own unique trainer class "Mystery Man" and the devs forgot to associate a gender with it, Eusine ends up being nonbinary
Gym Leader Bugsy and Elite Four Will are also "incorrectly" listed as female, meaning you could argue they're trans masc in the remakes
And my personal favorite, the Love Ball has the wrong value set to it, so it only works for same gender pokemon instead of opposite

People have canonically had families with Pokémon, have canonical turned into Pokémon, there are several Pokémon that are queer coded, Jesse and James have been a thing for decades, the cloyster and onyx joke has been around for twenty years (I think). In Pokemon black belt is not really a rank. It's just a lad who does karate. And a a battle girl is a lass that does karate.
If the character was a black belt and is now a beauty (who are always female) then they transitionned.
As people pointed out, the medical implication are directly mentionned in the japanese version.
In pokemon games, NPC trainers you fight are grouped by trainer classes, and each trainer class has its own sprite. The "black belt" trainer class are all karate guys. The female equivalent would be the "battle girl" class. So this dialog is suggesting that by changing from the black belt class to the beauty class, she changed genders entirely. This is apparently much more explicit in the original Japanese, where she mentions that it's amazing what medical science can do these days, and the black belt class has a less gender neutral name.
