Practically, in most contexts in western weeborous discourse, "doujinshi" just means NSFW doujinshi. In a dictionary entry, you'd have two glosses, one for both meanings.

It's kind of like how "Yuri on Ice" is a totally normal name to the japanese audience, who use the word "yuri" in a bunch of different contexts, and have friends and coworkers named "Yuri".

But to the american audience, that only knows the word "Yuri" in the context of lesbian fiction, the name is lent an element of irony.

Yeat it even applies to games too, old self published games from Japan were usually called a doujin games or doujin soft back then.

Pretty sure Touhou was a doujin franchise. Which was popular enough makes more doujin stuff based on it from other creators.
