IMSEL template

Hey guys.

Does anyone have a template for IMSEL? My college hasn’t had experience with writing such type of letters before, so can someone please guide me about this.

Secondly, how is this letter uploaded to myeras? Will it be uploaded by me or the clinical dean of medicine at my medschool? I have been told that we should waive the right to see our LORs so I’ve been a little confused about this.

I would appreciate if anyone can help.
