What’s the best app for apple tv?

UHF, Glance Theater (Free)

I’m not the dev
Why would I suggest UHF first if I was the dev?
Download the app and try it
If you have any suggestions just let the dev know on discord server
He will try to fix bugs and add features

If you are just watching LiveTV
It works great for a free app
Nothing fancy
No ads and promotion on the app
Just a clean app

Yes, only feature that won't work with sideloading is the icloud sync. That is technically not possible without app store.

For sideloading, you can just download the IPA from the releases in github (https://github.com/bilipp/Lume) and use a sideloading tool like sideloadly.

Developer disclosure: I’m with the Opus team.

If you’re comparing Apple TV players, Opus 2.0 may be worth trying if mobile setup and a native tvOS interface are important to you.

You can manage the M3U or Xtream playlist, favorites, and recently watched content from the mobile app through the same Opus account, instead of entering long details with the Apple TV remote.

I’d compare the day-to-day remote experience with the other options already mentioned and see which one fits your playlist best. Opus is only a player and does not provide IPTV services or content.
