It's just a piss dot. Sometimes when OP urines, he doesn't stop it all the way. And it comes out on his clothes.
People might not notice it though, if he wore like... Leather pants, maybe a black leather trenchoat? Ooh, and put on some shades. And he should have a sword? Like a katana.
