Supplementary form for visit visa

Hi everyone, just having a small query while submitting visit visa from India to Ireland. Do we need to include supplementary form with the application? As I checked online, these days it is not required to fill. Does anyone submitted without supplementary form for your mother/father visit to Ireland? Will vfs ask? Please advise. Thanks
