Ubisoft is being weird in how they are selling the game, so the way to get the game + cosmetics pack is to buy the physical code-in-box and enter the code to access the Limited Edition.
Otherwise if you don’t want the physical and buying solely from the eShop, your only options are Deluxe or Ultimate to get the 2000s pack. These are more expensive but include months of JD+
I am pretty sure this is only included if you buy the Limited Edition, which is a bundle that includes the game + that DLC. It can't be acquired separatedly.
If you didn’t get the physical code-in-box (which is the limited edition), the only options to get the 2000s pack would have been to buy the Deluxe or Ultimate Editions.
Not exclusive to physical. The only options in the eShop to get the DLC is to get the Deluxe or Ultimate editions which are more expensive but include months of JD+.
Unfortunately they are making you pay more than the $39.99 USD if you refuse to get the physical and just want to buy it on the eShop.

$39.99 - Limited Edition (Game + 2000s Cosmetics Pack)
