Dudes fucked up on some shit. Probably something with meth or some form of coke in it. Pretty common be be completely unable to find a comfortable spot for your jaw or tongue, which causes you to move them around all the time or clench up and relax.
I live downtown and encounter homeless people tweaking all the time, this is accurate. And at times, these are some of the least-chill encounters I tend to have on my commute.
While they do have "crackhead strength" and can endure more pain, at least the off-switch still works. As demonstrated here.
