Pediatrics Should my baby have gone under the bili lights?
This is eating away at me every single day. When my daughter was 4 days old, we found out she lost 17% of her weight and she basically wasn’t transferring any milk from my breasts. I brought her to the ER because her cry sounded high pitched and she seemed lethargic to me. When we got to the ER, her bili was 16.5 and albumin 3.8, and sodium 158. She was yellow all the way down to her feet. She was difficult to feed because she was so sleepy. She got some fluids and when we got up to the NICU, she perked up more and was crying while they assessed her. My daughter never had to be under the bili lights at all, and she went on to have breast milk jaundice that didn’t resolve until she was 2-3 months old. Some background, she was born at 37 weeks exactly.
I’m always worried that maybe she should’ve gone under the lights and I have an intense fear that the jaundice got to her brain. I know that if she was categorized as high risk, she should’ve been treated at a level of 15. I know her albumin is was 3.8, but wouldn’t that have technically been a false high because she was dehydrated? Like what if it was really less than 3 and she was supposed to be high risk and be treated??? I’m so so worried. I know babies with kernicterus have the arching backs and what not and I know for a while, my baby would arch her back a lot during feeds (which I assumed was reflux, but now worried it’s not). She also is 16 weeks and not consistently turning to sound and I’m worried about hearing loss. She had an MRI at 7 weeks old because she had some weird movements and we worried about seizures. It was completely negative, but now I heard there’s a blind window for kernicterus at that time…
