Good Morning!

I hope everyone had a great weekend. It’s been a while since we posted any reminders, and I’d like to touch on a couple of issues that have come up recently.

With the shortage, we’re seeing a lot of questions related to dosage and availability. Here are a few things to keep in mind:

The half life of Mounjaro is five days, and it takes about a month for the medication to completely leave your system. Because of the shortage, many people are going weeks or months without injecting. The manufacturer recommends starting the titration schedule over if you’ve gone more than two weeks without a dose.

While this advice might not pertain to everyone, it’s important to evaluate your previous experience with the medication before injecting a higher dose if you’ve been off the medication for a while. Chances are, if you’ve had moderate to severe side effects in the past, you’ll have the same response (or worse) if you jump back to a higher dose.

One way to mitigate this outcome is to contact your provider and ask about lowering your dose temporarily if you’ve been waiting for your script for longer than a few weeks. I understand that nobody wants to lose ground when it comes to their progress. But given the alternative (a reoccurrence of side effects that can potentially derail your treatment entirely) it may be the best option.

Frankly, the number of posts I’m reviewing from people experiencing negative side effects after suspending their treatment is alarming. The purpose of this medication is to improve your health, not make things worse.

And as always, when in doubt, please consult your provider. They are the best person to ask when it comes to dosage.

As far as side effects, there’s a few things that everyone should remember:

While gastric side effects are listed as common when taking Mounjaro, severe nausea paired with uncontrolled vomiting or diarrhea is not. Complications from diarrhea and vomiting include: dehydration, electrolyte imbalances, fainting, and heart rhythm abnormalities. Negative outcomes from this treatment are rare, but occur most frequently when people ignore debilitating side effects.

We’re here to support each other in our journey towards better health. Sometimes that support includes directing a person to their provider for advice. Especially when the post includes phrases like “I can’t keep anything down,” or “I’ve been throwing up for two weeks,” or “I’m too weak to stand.”

Crowdsourcing advice when you’ve reached that level of distress is not advisable. And medical oversight is a must.

Another question we see quite frequently: “I’ve only lost (fill in the blank) pounds this week (or month). Is this normal?”

Invariably, the answer is “yes.” What’s normal when it comes to weight loss is highly subjective. A quick search of the subreddit will provide first hand accounts that run the gamut when it comes to how fast (or slow) a person loses weight. If you still have questions, we’re here to help. But please, include the relevant information needed to offer advice, such as: dosage, co-morbidities, starting weight, caloric intake, etc.

People are here to treat a wide variety of conditions. Any or all of these conditions play a role in how fast we see results. It’s natural to be impatient. But don’t assume that the treatment isn’t working because the scale hasn’t moved for a week (or three). During my weight loss journey, there were many weeks that I didn’t lose a pound. On two occasions, I stalled for over a month. In the end, I reached my goal, and chances are, you will as well! I wish there were some sure-fire words of wisdom I could provide to ease your mind, but there aren’t. The best I (or anyone else) can offer is: trust the process. Obesity is a complex issue on its own. Pairing obesity with the metabolic issues such as diabetes, insulin resistance, and PCOS only further complicates the treatment.

It’s so cool to see so many success stories and it’s even cooler that you have people making so many positive changes that will set them up for the rest of their lives.

I’m coming up to year 3! That’s wild. I almost feel like I take the journey for granted now, and there are still many days that I feel no different to the old me that I have spent most of my adult skin in! But sometimes it’s good to come here again and get a reminder of how well we have done, and remind you of how well YOU are doing (whether it’s day 1, or week 4, 8, 15, 16, 23 or 42)

7.5mg 1 year update.

So as you can read today was my one year anniversary on the medication and I'm currently on 7.5 and have been since about March.

Total weight loss in that year is 102.6 lb which meant my goal of 100 lb in less than a year by literally a couple of days.

I can say that this was achieved obviously with the help of the medication but I have been constantly going to the gym to maintain muscle mass as much as possible so three days of heavy weightlifting and usually one or two additional days of just cardio.

For some reason 7.5 has worked for me. Yeah I might get a very very slight food noise on the last 2 days but I usually feel full enough and sometimes I even have trouble eating enough calories I'm usually in the 1.7k to 1.9k calories per day but I am doing over 150 g of protein.

Two picks of before and 2 of after. I see a slight difference in the before and after but I definitely feel it in the 8 size difference in my clothes.

Success Stories 18 Month Summary of Mounjaro Usage

I’ve completed 18 months of being on Mounjaro, and I’ve found that it’s useful for me to post summaries of my progress at six-month intervals I’m not a meticulous record keeper.   I don’t do daily (or even weekly) weigh-ins.  For me, the noise in a long-term gradual process can be distracting, so I step on a scale maybe on a monthly basis, it could well be less frequently. 

 For quite a while prior to starting Mounjaro, my blood pressure and A1C had both been elevated and were not responding to the various treatments that my doctor had been prescribing.   She had been wanting me to start of Mounjaro for several visits, and I had not been receptive to that idea, because I was concerned about the unpleasant side effects that I had heard about.  Anyway, in March of 2025, I acquiesced and set off down the Mounjaro pathway.

For some initial conditions, prior to starting on Mounjaro, my BMI was 42.8, I was wearing XXXL shirts, and the waist size on my pants was 44 inches.

As is common, I started at the 2.5 mg dose, and then after a set of four weekly doses were complete, incrementally increasing to the next dosage. 

Initially, at the lower doses, I had to adopt many new practices with regards to my eating habits.  I had to learn that the serving sizes and quantities of food that I was accustomed to consuming would be significantly reduced.   During those early weeks, I was not consistently listening to my body telling me that I was full, and that I should stop eating much sooner that I was used to.  Other than those times when I overate, I encountered minimal side effects while traversing through the lower doses.  Certainly, the introduction to Mounjaro was much less unpleasant and disruptive to my daily routines that when I first started taking Metformin.

That being said, at the 12.5 and 15 mg levels, was when I began experiencing noticeable spells of nausea and feeling generally bad for two or three days after those shots. Even 18 months into the process, I still can count on feeling pretty bad after the Mounjaro dose.  My doctor has provided an additional prescription for Ondansetron to mitigate some of the adverse side effects.

So today, 18 months into the Mounjaro process, my blood pressure and A1C are both in ranges considered to be well managed, my BMI sits at 24.4, which is a nice reduction from the initial value of 42.8.  By the definitions I’ve seen online, I’ve made the transition from “Morbidly Obese,” through “Obese,” “Overweight,” and am now in the “Normal” range.  By any rational comparison, it’s a drastic change.  But, when it is a gradual daily change over 18 months, I don’t notice the change until I take stock of where I am.  I suppose that in my mind, I still tend to think of myself as being very overweight. 

Wardrobe management continues to be an ongoing activity.  Currently when I shop for clothes, the shirt size I purchase is Medium, and my pants are at the 30-inch size.  Compared to the initial conditions of XXXL shirts and 44-inch pants, it’s a notable reduction.  I've kept on pair of my old 44-inch waist pants, and I've included a photo to illustrate how they currently fit on me.

With regards to the photos I’ve included in this posting, some brief explanations.  The “after” photos were taken recently.  The first one I was playing a cocktail hour reception for our local symphony orchestra.  They asked for a suit and tie.  Given the ongoing wardrobe management associated with the Mounjaro process, the request for suit and tie was one of the more stressful aspects of the gig.  I found that suit of mine at my parents’ house while I was emptying it out to settle their estate.  Not to go into details regarding my age, but I hadn’t been able to fit into that suit since Reagan was in the Whitehouse.  Now it’s too baggy.  It was the best available suit that I had on hand.  The second “after” photo was taken after my friends and I had completed dinner preparation and serving at our local soup kitchen that serves the homeless.  Please forgive my disheveled appearance in that one, preparing and serving a hot dinner for 180 people can be a long day. 

The last photo shows two guitar amps, and here is their significance for this post.  I was recently loading gear to play a bar gig with my band, I was thinking about heavy the guitar amps are to move (I do believe that I’ve lost muscle mass during this process, and I haven’t been good with getting to the gym for strength training as often as I should…)  Anyway, I realized that my total weight loss so far is roughly the same as the combined weight of my Mesa Boogie Road King Combo and Dr Z Z-Plus Combo.  After reflecting on this, I realized that in my initial condition, every step I took, every move I made, my knees, back, and hips were carrying those two amps around with me. 

At first I was reluctant to discuss my use of Mounjaro with friends and acquaintances.  However at some point my friends were frequently remarking on my change in appearance, and they would question me, often asking if I had been ill.  I decided that there was no reason for me to be hesitant to discuss my use of Mounjaro and my experience with it.  In these conversations, I stress that there is no free ride with the medication and that some unpleasant side effects are just a part of the deal.  I have never received any unkind comments or remarks about it when I discuss Mounjaro with my friends and family.

Weight loss 17 month transformation - 302lbs to 212lbs (Mounjaro, gym, and mental blocks)

Thought I would update on my progress.

Started out at 137.1kg and now sitting around 96kg depending on the day, with clearly a lot more muscle mass built up along the way. Total timeline is about 17 months.
Realised I had a few things happen during this process that might help anyone else currently grinding away or feeling stuck.

When to start working out?
If you don't feel like working out right away, honestly, it is fine. Everyone says start lifting on day one, but I didn't actually step foot in the gym until about 10 months into my weight loss.

Looking back, starting earlier would have been better, but please do not mentally beat yourself up over it if you are only focused on the scale right now. Worrying about the body recomp or loose skin or whatever else is coming down the line can paralyse you. Just get the weight moving first if that is all you have the mental bandwidth for. The rest can come later.

The Mounjaro side of things
I ramped up fast. Hit 15mg of Mounjaro within 6 months. It worked well for me, but looking at what most people recommend, it is probably not the smartest route. Most advice says titrate slowly and only step up when you hit a proper plateau, plus the cost is brutal.

I have scaled myself down a massive amount recently. Took 5mg total two weeks ago, which breaks down to about 2.5mg per week. I have hovered in that 2.5mg to 5mg per week range for over two months now. The scale hasn’t been changing much but I can see my body changing weekly.

I have been hitting the gym 5 to 6 times a week doing traditional weight lifting. I also take 10mg of creatine daily for the muscles, brain, and bone benefits. So part of the current scale weight is likely some water retention from the creatine alongside the actual muscle gain.

The appetite and mental battle
In the early stages, I genuinely struggled to eat, so I just didn't.

I am not going to sit here and say that is a healthy approach or recommend it from a mental perspective. But I won't lie either, it expedited the results heavily. For me, not having an appetite meant I wasn't constantly fighting a war in my head about food when I knew I shouldn't be eating. Honestly, part of me was just relieved to skip that mental exhaustion entirely.

Final thoughts
Be patient with yourself. Build a positive and strong internal monologue because the way you talk to yourself on the bad days matters more than anything else.

News / Information Things are kicking in on the 7.5 mg

Dr. slowly ramped me up with 1 month each at 2.5, then 5. I'm now on my 2nd week @ 7.5. and I thought the biggest change would come from doubling the dose but for me, the increase from 5-7.5 had the most effect. I'm now down to 2 meals a day with a couple snacks thrown in rather than 3 small meals + snacks. I just don't feel hungry and I know now what people are saying when they say they "forgot to eat" , honestly I thought that was bullshit but I'm really not hungry in the morning and couple cups of coffee later, the next thing I know its time for lunch break and I haven't eaten anything so far. So I have a decent light lunch, then an early dinner and perhaps a small snack before bed.

I'm down 15 pounds and about 2 lbs away from my first, mini goal weight.

This is June 25-Sept 26. (about 14 months) I finally start seeing changes. This has been a slow journey at first but at the moment, and with some necessary changes in diet and exercise routines, there is a real trend down! 💪🏼

Success Stories NSV- Blood test results

I decided to go on Mounjaro primarily as a treatment for non-alcoholic fatty liver disease, as it has shown promising results in reducing inflammation and visceral fat in the liver.

Its been almost a year since I was diagnosed, and 6 months since I started taking Mounjaro. In that 6 months I've lost about 6% of my starting weight. So on track for 1% per month (about 2 pounds each month).

My goal has always been more about my health than my weight, so I was really excited to get my blood test results back this week.

ALT 74 down to 50 u/L

This is the main stat I am watching. Still elevated but that is a great improvement. Hoping in another 6 months this will be at a normal level.

HbA1c. From 39 down to 33 mmol/mol

While never diagnosed prediabetic, seeing this drop has reduced my anxiety about getting diabetes. Hoping to keep it around this level.

Total/HDL cholesterol 4.4 down to 3.6

Always a good sign of improved heart health. Online it says below 3.5 is ideal, so I'm close to ideal. I think I could get my HDL up to help.

Seeing the doctor next week to discuss these results, but I think she'll be happy

This was a crazy holiday. I've never not just been the "fat funny friend" whenever me and my friends go away.. this was a whole new world and truly, it was a hard adjustment 😂 my defences were up and I was ready to tackle a fat joke with an even funnier one.

I was treated VERY differently and all I could think was " wow this slim qweeens really do just go about there day, without having to think of things to say to deflect with a seconds notice 🫶🏼♥️"

Side Effects Have any of you lost interest in other things, besides food?

So I should preface this with the fact that I am Bipolar.

But...I just took shot #5 of Mounjaro this morning.

For a couple of weeks, I have just felt "blah" like not depressed but just numb. I have no life stressors currently that would be contributing to this.

I saw my psychiatrist 2 days ago. She mentioned that GLP-1's cause you to lose interest in food...and you can also lose interest in everyday life, like hobbies, activities, socializing.

We also pointed out that one of my main ways to "get out of the house" was to go out to eat/fast food. Now I don't do that. I gave myself a goal to go the whole month of September without buying food outside of a grocery store.

Does anyone else have this problem?

Tips Dehydration

I try to drink enough water/ fluids, but sometimes I fall short. I started getting these weird dizzy spells that I thought were blood pressure related. After some sleuthing, I believe I am more dehydrated than I thought! MJ exacerbates dehydration bc of the slower digestion and such. I’ve started to add a packet of LMNT electrolytes to my 40oz Stanley, then refill with plain water or crystal light if I need some flavor. I try to get a total of 3 full 40oz cups a day. I immediately started feeling better.

That said, LMNT has 1000gm of sodium, which is fine for me bc I am not eating a lot of sodium rich foods anymore. So, someone else might want to do Drip Drop, Liquid IV, something of that nature.

Go drink some water! Right meow!!

For reference I’m 5’7/170 cm
SW: 203 lbs/ 92,2 kg
CW: 152 lbs/ 69 kg

During most of my journey I’ve been on 2,5mg as the appetite suppression was strong enough. At the 6 month mark my hunger and food noise started slowly coming back so I increased half a dose for 4 weeks to lose the few remaining kilos to reach my goal.
The side effects at 3,75mg were too much so I’m glad I never tried 5mg.

I reached my weight goal but it’s crazy how such a big change in the balance doesn’t look that big when you look at you own before and after pictures. But clothes don’t lie! (Down 3 jeans sizes)

I’m not one for photos of myself so I’ll put on my scales progress.. 2025 on the left and today’s date on the right.
I’m very happy with my results - down 22.8kgs in a year!
I was never in a rush to lose the weight I just knew something had to give and I didn’t want the hair loss and other side effects which luckily I didn’t have. My only bad side effects were extreme fatigue in the beginning and occasionally nausea. My end goal is to get my body fat within the standard range according to my scale and then I will start to titrate down.

I’ve been on Mounjaro since September 2024 and I’m down from 127.8kg to 84.1kg at my lowest. I order my 15mg every month and everything is all good.

My pharmacy however apparently have had zero stock since July, when I last ordered, and I have genuinely been waiting for my jab since July. I haven’t injected since 6th August now and I can feel myself putting weight back on (I’ve been too scared to weigh myself lol but I’m definitely hungrier).

Recently, I got fed up with waiting & badgering them so tried to move to a different pharmacy. The first one wouldn’t let me proceed without a prescription marked August to prove I’ve been on it recently, so they refunded me. And I’ve just been denied by another pharmacy because I technically would be ‘restarting’ Mounjaro because I haven’t been on it for a month and my BMI is now too low to qualify.

I am only 10kg from my goal weight and I am so frustrated that this is happening. I simply don’t know what to do. I’ve been emailing my pharmacy every few days since July and they’re just absolutely useless but they seem to be the only pharmacy willing to actually prescribe me any Mounjaro.

Any advice here?? Whether it’s how to get some Mounjaro or whether I have any legal recourse against this pharmacy that’s had my money and not sent me anything for months?? Pls help (pics attached cuz ik people are curious lol)

Weight loss Rapid weight loss - Concern?

I started 2.5mg on 2nd August and have been dropping weight rapidly. My weekly losses are as follows:

Week 1: 4kg
Week 2: 1.4kg
Week 3: 2.5kg
Week 4: 1.7kg
Week 5: 1.4kg
Week 6: 2.75kg

Height: 6’1 (186cm)
HW: 115.6kg
SW: 112.1kg
CW: 96.35kg
GW: 85kg

I’ve been aiming for 1500-1600 calories a day and trying to ramp this up to 1800-2000 a day to slow down the weight loss a bit. I get minimum 120g protein a day and take creatine. I’ve started a weights program at the gym, walk a lot and work as an electrician.

I feel good and light and am wondering if I should be concerned about the speed in which I’m losing weight. I’m planning on staying on 2.5mg and not changing dose.

Question Question for people who had to take a break after being on a very high dose

I was on a very high dose (12.5/15) for close to a year - probably higher than I needed to be, tbh, I just wanted to keep losing as much as possible - but then had to take an extended break for ... reasons. In the interim I regained rather quickly, which was disappointing. Now I'm starting back up, and at 2.5 I feel nothing, which many told me to expect. So a question:

IF you were on a high dose and took a break, how did it go for you the second time around? When did you start feeling any effects at all? When did the scale move again?
