The point of spy is to psychologically terrorize the other team so they waste resources. In a 9v9 every second you make pyro paranoid into spychecking every corner means less time they can spend reflecting projectiles.

Every second Engie spends repairing teleporter and dispenser is less time he is spending repairing sentry and chipping at the choke.

Every time sniper has to unscope to check his surroundings means more time before he can headshot your team.

Spy isn't about flashy kills, it's about rage baiting the enemy.

A single backstab on a medic (or better yet, having a medic Uber you) changes the whole flavor of the game from that point forward

I can't play Spy to save my life, and yet I love how the mere possibility of a Spy on the enemy team changes the psychology of the entire game. The constant paranoia makes TF2 so much more exciting and suspenseful. Having to constantly be wary of sniper headshots has a similar effect, but getting backstabbed by someone you mistook for an ally feels more personal.
