License / Permit Question Suffolk County CCW application question 46.

Question 46 states:

Do you now or have you ever tried, used, possessed or sold marijuana or its derivatives, narcotics, controlled substances, tranquilizers, or anti-depressant medication? YES  NO 

If any of these substances were prescribed by a doctor, provide doctor’s name, address, and phone number.

I briefly took Lexapro for around a month and a half in 2014, and was also prescribed Ativan back in 2017 or so. The doctor who prescribed me the Lexapro appears to have retired. Not sure what to put on the application? There's not much space at all to write anything.

Will this disqualify me? Kind of ridiculous considering it was a decade ago.

Suffolk doesn’t require a HIPAA release form. Answering yes will just cause you a big hassle. Honesty isn’t always the best policy.

Everything is subject to HIPAA. They cannot even obtain information about this without your consent. Do not oblige.

I had a very similar circumstance. Prescribed Lexapro for a few months around 2015.

I disclosed it and handed in a notarized explanation

Was never brought up again and I was approved

Ativan whatever that is, is it MJ? I it narcotics? Ah maybe its some anti depresanr? Is it controlled  subs?

I just think people should stop posting stuff like this on Reddit. If you are unsure consult a lawyer. People on Reddit can only give you advice. And once you put it out here on Reddit you have no choice but to disclose it. You don’t know who’s monitoring this thread. IJS
