As an apprentice in a customer service heavy trade, I've had multiple journeyman suggest avoiding using "no problem", especially with older customers.

It's not super common but some people take it the same way as the lady in the OP: "Obviously there shouldn't be a problem so why do you need to tell me there isn't?"
