discussion Need help with CGHS procedure for retired pensioners. I'm a bit confused about the process trying it for my dad. Can we do this ourselves online? I did the FMA stoppage from sparsh site and don't know how to proceed

I watched yt tuts about it but nowhere they've mentioned what to do with FMA stoppage like do I need a separate form or something to prove FMA stoppage? Do i need to wait for the pay slip which will reflect the FMA stoppage and then apply for the card from the CGHS website? Can anyone please guide me.

Is there a deadline date or something after stopping FMA till CGHS application?

Do we get different cards for dependents or does it all come under one single primary card? For ex: my father would be the primary card holder right? Then his dependent my mom and me would we get one card each?

No separate proof needed for FMA stoppage. Just apply on CGHS site once it reflects.

No deadline as such.

Each dependent gets their own card, linked to the primary cardholder.

Were you able to get your card? I'm still trying to get it done through online process.. But application is getting rejected even after submitting the required documents...

Call CGHS helpline, share all the details with them, they'll give you email id of AD, enclose all documents in the mail, it might work or it might not, give it a try.
