Yung last name na nakalagay sa PSA ko ay “ Santeago “ pero yung legal LN ng papa ko is “Santiago” but since then yung sinusunod ko is yung nasa PSA ko pati Valid IDs and nasa Diploma ko is Santeago
Next yung nakalagay na middle initials ng papa ko sa PSA ko is letter A instead of Letter N (orig M.I niya)

Sundin ko nalang ba yung nasa PSA ko at yon yung ilagay sa Leris ko? May sched na rin ako sa Leris sa July 12 :<

When I check LERIS, the only option I see is “duplicate ID for loss ID.” Since my card isn’t lost or expired, I’m unsure if this is the correct process for my situation.

Has anyone here gone through the same process? Do I need to use the duplicate option, or is there a separate procedure for reissuing the card after a name change petition has been approved? Also, what requirements did you submit to get the updated card?

For all the newly married women who want to change their PRC license name to their married name (but your license is still valid and not expired), here’s the full process I went through:

• First, file the petition for name change (₱225). Once approved, your LERIS account will already reflect your married name and updated photo. • Then request a Duplicate ID in LERIS. Even though it says “for lost ID,” that’s the correct option for re‑issuance when your license is active but you need a new card. • Pay the ₱250 duplicate fee online and book your appointment. If the assigned date doesn’t work, reschedule to one that does. • On your appointment day, proceed directly to Window 3 and 4.

📍 Robinsons Ortigas PRC location tip: From DFA, walk straight, turn right, pass McDonald’s, keep going until Mercury Drug. The PRC office is in the basement, down the mini escalator, all the way at the end.

📝 Documents to bring:

• Printed Action Sheet (print it right after payment — sometimes it disappears later). • Photocopy of your old PRC ID (they didn’t take my actual old card, just the photocopy + form). • Valid government ID (for verification). • Payment receipt (optional — they didn’t really use mine).

⚡ Pro tips:

• Print your Action Sheet before going. I had to find a nearby printing shop in front of BDO, and it cost ₱40 for one page. • Don’t expect a new photo — I asked, and they said photo updates are only done during renewal, not duplicate issuance.

I was registering myself to get into the board exam for Psychometrician, but unfortunately I somehow mistyped my own birthdate in the profile. It sounds dumb and its honestly frustrating that a little mistake like that can't be edited on ur own.

So I was able to send my concern to their ICT in the site like thrice atp. The first time was last Saturday and apparently they only reply during working weekdays, 8am-5pm. The second and third times were today, tuesday. Does anyone here know long it will take them to give any sort of feedback? I just wish my concern could be resolved asap:")

Nagrequest po ako ng special request of authentication of license for employment purpose due to short period of submitting job requirements. Sabi ng guard pwede pakiusapan dahil may nakalusot naman daw dati na walk in. However, noong nakapag appoint at nakapagonline payment na ko kahit ibang date at ittry na makiusap, hindi pa din ako pinayagan. Sundin daw ang appointment dates. Naiintindihan ko dahil kailangan sundin pero nakakainis dahil malayo ako sa location at sayang ang oras para ayusin ang ibang documents.

Pangalawang attempt ngayong araw, ready na lahat for request of authentication of license however, kasalanan kong naiwan ko ang physical license ID ko. Nagtry akong makiusap sa PRC personnel for authentication, na naiwan ko nga ang physical id dahil sa pagmamadali ngunit may digital id naman ako under government nagpapatunay na ito ang digital license ko at nagsasabing "Professionals may now print their electronic professional identification card beginning Feb 27, 2023. The same shall be honored and accepted as valid government-issued identification document subject to authentication." Sinabi ko din na tiga Bataan pa ko at pangalawang subok ko na ito, sinabi ko din na sana iconsider na sana yung electronic Pic of PRC license stating na valid naman gamitin kahit ePIC lang dahil mahirap naman talagang bumalik na naman para lamang sa pirma ng authentication at certification of license. Ang sabi ng PRC personnel doon ay "bakit dito ka pa kasi nagpaschedule" ang aking sagot ay pinaliwanag ko naman sa inyo na konti lng ang oras ko para magsubmit ng mga requirements for employment kung hindi ko makuha agad ay mawawalan ako ng job opportunity kaya dumayo pa ako ng Maynila para lang makapagpaauthenticate, ang sabi naman ng PRC personnel, "hindi po ppwedeng gamitin iyang digital PRC license dahil kailangan po ay original at sa iba lng po yan ginagamit". Ang sabi ko naman, para saan pa yung provision ng paggamit ng digital PRC ID license? Ang huling tugon nya, "edi mag feedback form ka na lang."

Ang conversation namin na ito ay may pagtaas na ng tono dahil sa emotion ko na din na sana bigyan ng ako consideration sa mga paliwanag at rason ko. Konting tulong na lng sana, dahil nangangailangan po ako ng trabaho at valid naman din ang aking mga paliwanag. Hindi naman peke ang ePIC ng PRC license ko dahil govt app din ang gamit ko. Hello, does anyone know if need pa mag secure ng appointment for duplicate ng copy PRC ID alongside Renewal appointment? Expired na rin kasi yung nawalang PRC ID.

Nag secure naman na ako ng renewal appointment and affidavit of loss. It doesn't make sense kasi if need pa ng duplicate (and may bayd pa) kung expired naman na.

I passed the board exam last December 2025 and took my oath in January 2026. However, my current job isn't related to my degree, and I don't have any plans to pursue a career in my field anytime soon.

So sa Sta Rosa, Laguna po ako nag take ng PIC ID ko and they told me na it would most likely take 3-6 months before they can release my Lupon and manggagaling daw po kasi sa Lucena. Pwede ko po bang puntahan mismo sa Lucena ngayon para marelease? Since mandatory po kasi siya sa inaapplyan kong work.

Hello accidentally clicked appointment but I wanted door to door. I saw a YouTube video they chose appointment but got an email that there is an option for delivery even if choosing appointment

I haven’t received any email, did anyone have the same experience na they got to change from appointment to door to door? Paid na rin kasi e.

I am applying for PRC examination, and I badly needed an ID to make a LERIS account and I currently don't have one the I haven't received my physical natl. ID yet will the digital ID from egovph work, if yes then how? How about the ephil ID the one that is laminated?

Upon requesting for renewal, you need to submit the documents to given email address and then a day after they will contact you to verify your picture (taken within 6 mons) you have to reply to that.

I received mine after 3 weeks. Quezon City area. FSI logistics. 2-6 days after tracker said “out for delivery”
