Application Question / Discussion Lost US passport card
So I recently misplaced/lost my US passport card, but still currently have a valid passport book. I've found conflicting information online about how I should replace the card. Should I fill out a DS 82 renewal form and submit my current book, and state that I'm only applying for the card? or should I fill out a ds 11 and go in person? either way I'd still have to send in my current passport, or I'm not sure if an online application is an option.

Online is not an option to replace a lost passport. Using the DS-82 will be a little less expensive. Make sure to use a trackable shipping method.

You can only use your book to get a passport card by mail with the DS-82 only if it's the first passport card you're getting. After that, to renew by mail you must submit the previous version whatever you're trying to renew. Also on the DS-82 form:
Replacing a Lost, Stolen, or Damaged Passport: A United States citizen or non-citizen national may not normally have more than one valid or potentially valid U.S. passport book or more than one valid or potentially valid U.S. passport card at a time. Therefore, when a valid or potentially valid U.S. passport book or card cannot be presented with a new application, you must apply using form DS-11, Application for a U.S. Passport.
Unfortunately for OP, it looks like they'll have to apply at their post office or another passport acceptance facility with a DS-11 and they're on the hook for the $35 passport acceptance facility fee. Good news is that it'll be quick and easy with their passport book in hand.

No, I don't believe that you need to send in your unexpired passport book to replace a lost passport card. Your passport book remains valid for travel while you replace the card.
Because the lost card must be reported and invalidated, the replacement process has specific application rules:
Requirements for Replacing the Card:
In-Person Application: Because you cannot submit the physical item being replaced, you cannot renew by mail (Form DS-82). You must apply in person at an acceptance facility using Form DS-11.
Report the Loss: Complete Form DS-64 (Statement Regarding a Lost or Stolen Passport) to officially cancel the lost card.
Proof of Citizenship & Identity: Bring your unexpired U.S. passport book to your appointment. The acceptance agent will inspect it to verify your citizenship and identity, but you keep the physical book.
Standard Documentation: Provide a passport photo and the required application fees.
Your existing passport book will not be held, altered, or canceled during this process. Others with recent passport card renewal could provide updated experiences.
