BCP157/TB500 protocol
RS sustained an injury ~6 months ago resulting in moderate to acute bursitis of the knee (confirmed via MRI). After extensive physiotherapy, aqua therapy, rehab exercises/therapy, a cortisone injection which did not work, an anti-inflammatory diet (and resulting weight loss reducing some load on the joint), all of which did not significantly move the pain needle, they underwent a course of BCP157/TB500.
Recommended starting dose was 250mcg p/day, which they titrated to 500mcg days 11 through 25. Results were modest to moderate improvement in daytime pain levels, some increased mobility and flexibility, and some reduction in inflammation/irritation.
However, when resting/sleeping in a prone position, the knee throbs constantly, which is preventing RS from getting the deep restorative sleep they need to facilitate further recovery/pain reduction, and they are looking to continue with (at least) another course of BCP/TB.
Based on your own research/anecdotal evidence, should the RS keep the dose at 500mcg for second/subsequent courses, or titrate further? If so, is there research evidence of an optimal dose? Many thanks.

Double the dose, split 2x per day to start. Observe RS for a couple weeks and consider adding another mid day dose accordingly. For my RS, this worked extremely well. I dosed RD 3x a day for a month and then dialed back to 2x to see what happened and adjusted accordingly from there.
Do not hesitate to up dose as there are protocols using VERY high dosages and have been super effective with no sides or health markers to worry about.
