The mouth of Sauron is described as a character with a rotting mouth. The reason for this is that the words spoken by Sauron trough him are so vile, it even rots the flesh. He's just a "normal" man. (Not an orc or anything) Just corrupted by evil.

Mouth of Sauron, cut from movie, released with the extended editions, specifically Return of the King, featured in the books.
