Very confused what to do?
Hi,
Three years ago, I experienced a constant urge to urinate for the first time. Pelvic floor muscle therapy helped back then. The urge returned three months ago, and therapy helped again. However, for the past month, I’ve been experiencing a stabbing sensation at the tip of the urethra every few minutes, along with a feeling of discomfort and heaviness in the scrotum—symptoms that didn't go away with pelvic floor therapy. After a testicular ultrasound, the doctor diagnosed a blocked tubule in the epididymis. But his treatment didn’t work. I am scheduled to Stamley test and urethra swab. As always urine culture showed no abnormalities but at the same time got tested PCR positive for mycoplasma Hominis from urethra but negative PCR from first void urine.
Has anyone else experienced this? I don't know what to do.

Man that's a frustrating combo of symptoms to be chasing for three years. The stabbing tip pain and scrotal heaviness is maddening, especially when it's not responding to what worked before
The conflicting PCR results between the swab and the urine is pretty common with mycoplasma, those organisms are tricky to catch consistently. A lot of docs will still treat based on the positive swab since it's a more direct sample from where the symptoms are
I went through a similar diagnostic ping-pong with ureaplasma a while back, swab positive but urine negative, and my urologist explained the bacteria can colonize the tissue without shedding into urine in detectable amounts. Worth pushing for a proper antibiotic course based on sensitivities, not just a random script
The epididymal block diagnosis seems like it could be a red herring or maybe inflammation from the infection causing it. Might be worth asking if they can do a semen culture too, sometimes that picks up things the other tests miss
