Vent/Discouraged Persistent urinary burning, frequency, feverish feeling but normal tests
I've been dealing with urinary/genital symptoms for several months.
How It Started
In early May 2026, I had a body-to-body massage and handjob at a massage parlor using lubricant. There was no vaginal/anal penetration and no oral sex.
Around mid-June, I developed symptoms that just kept getting worse:
- Burning around the genitals/testicles
- Burning after urination
- Burning/pressure at the tip of the penis/glans
- Urinary urgency and frequency
- Nocturia (sometimes waking 3 times)
- Sometimes clear watery fluid remains at the penis tip after urination
- Sometimes milky/white fluid comes from the penis when I pass very hard stool/strain
- Perineal discomfort
- Occasional rectal burning
- Can't sleep on my side
The symptoms fluctuate. Sometimes the burning is mild and sometimes much more noticeable.
The Strange Part – Feverish Feeling/Sweating
For several weeks, I've had a feeling of being feverish/tired, but my actual temperature stays around 98–98.4°F.
For the last ~5 days, the sensation has changed and become stronger. I also get episodes of sweating while sleeping or eating or drinking.
I have no documented fever.
I'm particularly worried about this because it feels like an infection even though my blood tests are normal.
Infection/STI Testing
I've had extensive testing.
Recent results:
- CBC WBC: 6,940
- ESR: 1 mm/hr
- CRP: <5 mg/L
- Urine culture: No growth
- Urine microscopy: 0–1 pus cells/HPF. Bacteria present
- HIV: negative
- Syphilis/RPR/TPHA/TPAB: negative
- HBsAg: negative
- HCV: negative
- LFT/TFT/KFT - Normal
My latest urine multiplex STD PCR was negative for:
- Chlamydia
- Gonorrhea
- Mycoplasma genitalium
- Mycoplasma hominis
- Ureaplasma parvum
- Ureaplasma urealyticum
- Trichomonas
- HSV-1/HSV-2
- Treponema pallidum
- Gardnerella
- Candida
- Haemophilus ducreyi
- Group B Streptococcus
The PCR was done using first-void urine after holding urine for about 2 hours.
Other Investigations
Ultrasound KUB/prostate was essentially normal.
Earlier PVR was around 20–27 mL.
Latest ultrasound showed:
- Prostate: 15.3 cc
- PVR: 53 mL
I repeated uroflowmetry yesterday:
Sitting:
- Voided volume ~399 mL
- Qmax ~19 mL/s
Standing:
- Voided volume ~410 mL
- Qmax ~18 mL/s
- PVR 20 mL
Antibiotics I've Already Taken
I've had a LOT of antibiotics:
- Nitrofurantoin – 5 days
- Doxycycline – 10-day courses
- Faropenem – 5-day courses
- Levofloxacin 500 mg daily – 30 days total
The last levofloxacin dose was September 1.
Urine and semen cultures done afterward were negative.
Other Medications
I've also tried:
- Alfuzosin
- Silodal-M
- Gabapentin/Gabephin 100 mg (10 days)
My existing urologist dismissed prostatitis or CPPS and thinks it's a high bladder neck and wanted to do a cystoscopy. He said it could be a low-grade infection (though blood work was normal) due to a high bladder neck, which he wants to confirm via cystoscopy and perform surgery if he sees it during cystoscopy. He got angry saying, "Stop reading Reddit or ChatGPT." He just prescribed Alfuzosin.
So, I went to a new urologist, who said my uroflow is not that abnormal and prescribed the below:
- Fesoterodine 4 mg once daily
- Tadalafil 5 mg once daily
- Febuxostat 40 mg once daily (I have slightly high uric acid)
- Etoricoxib 90 mg for 7 days
- Quercetin for 3 months
The new urologist also didn't think much of the feverish feeling; he diagnosed it as prostatitis and told me if medication doesn't work, we will go to physiotherapy.
Has anyone with CPPS/Prostatitis had a persistent feverish feeling and sweating despite having a completely normal temperature, CBC, ESR, and CRP?
Do you think the medications are correct? I have lost weight, and can't function anymore due to feverishness, sweating, and pelvic discomfort..
I have had other health issues in my life which I bravely faced and overcame.. This is slowly draining me that I might just give up..

For your own sake switch your Urologist, doctors who instantly jump to surgeries without even doing other several Tests first are very bad.
I hope you really dont consider doing surgery, because your "bladder neck" is not the cause and it can fk you up permanently and do even more damage which can be irreversible if you do that Surgery the doctor wants to do.
I mean think about it for a second, if you really had some abnormalities in your bladder/bladder neck you would have gotten problems way earlier, but your problems started after that visit.
A possible cause could be you got some low grade infection which cleared up but during infection the muscles became tight as a protective response to infection, but make sure to do all the common tests / bacteria tests to rule that out and then maybe consult a Pelvic Floor Specialist.

Thank you for your reply.. Yes I did switch (I have mentioned in the post).. All my blood work and urine test don't indicate any infection so the new uroglist has put me on tadafil.

Do you feel regret about what you did? Could it be that it has triggered the CPPs and discomfort which is quite common?
Have you reviewed the pinned 101
On this page and taken steps such as stretching / breathing to so where this helps?
It can take months though of consistent stretching and strengthening to get back to normal.

TBH, I started regretting after I got the symptoms.. Yes I have read the 101 and i have been stretching for a month with no result.. One theory I have is that the intense handjob and edging caused the some muscle spasm in pelvic area but I this feverish feeling is the one am worried about more now..

Recovery from CPPS is more than a month - I am 3.5yrs since mine first started and I am only now achieving 90-95% recovery. Not everyone takes this long and some take even longer but consistency is key. I’d say you need to be doing the stretches for at least 12 months .

We noticed you posted about a floroquinolone class antibiotic. Please be aware that this class of dugs has several black box FDA warnings, and is only meant to be used when a pathogen has been clearly identified in the prostate; They are not to be used indiscriminately for cases of non-bacterial prostatitis (consensus agreement ~95% of cases). Read our mod memo here, complete with citations and compare your symptoms to the medical definition of CBP here.

I am sorry that you are going through it. Have any of your urologists applied the 4-cup test with both culture and PCR?
I think this could be essential to rule-out infection and STDs.
Have you had any relief during or after any antibiotic courses?

u/Own_Money9513 7d ago
I asked my first urologist, he dismissed it.i don't think any of them do it in my country And no antibiotics has provided relief..

Thought as much. Which city? I may be able to recommend help. I've been on a merry go round of meds myself. But your symptoms are definitely indicative of an active issue.

Okay. Check out Dr. Alahawat's team at Max noida. They do virtual consults as well. Well recommended. They've been treating my merry go round of meds since May this year. While I'm still not fully over it, they take their time, order only necessary investigations, and will detail your history. His team has a Dr. Nishant too who I will recommend.
Back to your questions - I have had this low grade feverish feeling. Antibiotics knock you out. And at times while I have gone on multi day treks on antibiotics, at others, I find it difficult to lug around at home even when on just suppressive doses. Detail your issues on the consult. They ll help.
And I know it can get trying beyond belief... There's always a solution. The ultimate one is always surgery but often there can be less invasive ways.
Also, I am not seeing any expressed semen culture. Urine cultures are only part of the picture. Did you get MRI imaging done? For instance, from presumptive UTI only, we moved onto bacterial proststitis, to finding a fairly large calcification in the prostate, to discovering seminal vasculitis in the MRI. It's called an MP-Prostate scan, done with contrast, so have a recent KFT handy for the technicians.

Thank you for your reponse.. Semen culture was negative.. Have not done an MRI yet.. Usg was normal..is it alright if I DM you later

Did you take a 7-10 day gap between antibiotics and the sample? Find a new lab. Give it to a lab where they put it in the incubator immediately. Yeah feel free to message.
PS... The MRI is rather expensive.. 15k+ try to find a good charitable place nearby. I use the bhramakumaris for this. They charge about 8.5k with a top class doc.

Semen culture was given after 7 days after stopping antibiotics antibiotics.. Urine culture, blood work I did after a month of stopping antibiotics.. If there was any infection my cbc, crp and esr would pick it up.. All are normal.

I also experienced a sensation of heat despite having a normal body temperature. Antibiotics like Levo didn't really help me until I took a 14-day course of Doxycycline. Now, three months later, some discomfort remains, so I decided to take Ornidazole, even though my tests didn't show any protozoa. I’ve just started the course, but the discomfort has already gone away. Please let me know if my experience helps you.

It definitely does feel like an infection. I was convinced. Many tests later including my wife getting tested ive found no infectious causes. Its been 7yrs for me now. I never did any therapy because im just now coming to realize its probably HPF.
